Description

Position Summary: This position engages long term with all members of disadvantaged families to help them assess their strengths, set goals, develop plans to achieve their goals, and assures that all services that are provided by WCOH along with others the family might need, are 'wrapped-around' in a coordinated and effective manner.

Essential Job Functions:

* Works with WCOH program directors to recruit families into the Family Success Program;

* Builds positive relationships with families with whom they interact.

* Facilitates parenting classes for families using preferred curricula.

* Guides families through self-assessment of resiliencies/strengths, helps them conceptualize their goals, and supports families in creating plans for achieving goals.

* Interacts with families on a regular basis to provide encouragement and support, facilitates self-reflections, and conducts skill building with family members as appropriate.

* Refers families to partner agencies for family counseling/therapy as needed

* Monitors family progress and files reports as required.

* Embraces and embodies the mission, vision, guiding principles, and goals of WestCare Foundation.

* Performs any other duties as assigned.

Qualifications

Minimum qualifications

Education:

* Masters degree in social work, sociology, psychology, or related filed; prefer LISW credential,

OR will substitute

* Bachelors degree in sociology, social work, psychology, family studies or related field, with at least 5 years experience working in non-profit organizations with underserved families in urban settings.

Experience and Competencies:

* At least four years experience with demonstrated ability to work as part of a team delivering care coordination and wrap-around services to families or individuals.

* Must be empathetic, compassionate, non-judgmental, enthusiastic, highly organized and capable of managing a large volume of activities and documentation.

* Multi-culturally competent.

* Prefer Spanish speaker.

* Excellent oral and written communication skills are required.

* Must be computer literate in MS Word, Excel, Outlook.
