Salary

Base Salary: $58,256.71 - $67,455.14/annually

The expected compensation range for this position represents a good faith estimate for this position and the specific compensation offered to a candidate may vary based on factors including, but not limited to, the candidate's relevant knowledge, training, skills, work location, experience, and/or FTE.

About Us

Canvas Health's mental health and substance use disorder providers, clinicians, and staff bring their diverse lived experience to help inform and guide the care they provide to clients. Our integrated programs and services offer coordinated care to people of all ages in their homes, the community, and in our clinics. We strive for excellence in clinical care as we serve those living with mental illness, substance use disorders, crisis, unstable housing, and trauma. Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid.

Position Summary

This position provides planning, coordination and monitoring of mental health services to adults with serious and persistent mental illnesses who are eligible for Rule 79 or Minnesota Community Healthcare Network case management services. It is collaborative in nature and requires working closely with Community Services, Court Services, private providers, outside resources, Healthcare organizations, and programs at Canvas Health.

Schedule

Monday through Friday, 40 hours/week

Minimum Qualifications



  • Bachelor's degree or higher in the human services or social work field
  • 2,000 hours of supervised experience in the delivery of clinical services in the treatment of emotionally disturbed children and/or adolescents.
  • Two years' experience preferred working with mentally ill and/or chemically dependent individuals, of which at least one year is experience in community-based mental health, chemical health treatment with adults recovering from mental illness/chemical dependency or combination of equivalent experience and training.



Preferred Qualifications



  • One year in a case management or similar role
  • Experience working with and/or delivering services to diverse populations.
  • Experience and specialty training in crisis intervention and/or working with SPMI clients within the past two years and
  • knowledge of local social and human service networks
  • Experience with Electronic Health Record systems
