JOB TYPE Classification: Nonexempt Supervises Positions: No JOB SUMMARY The pharmacist serves as part of the healthcare team by supporting authentic personalized relationships with patients and their chosen support system, collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health. The pharmacist accepts responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures. Technical Competency: Provides optimal medication management and promotion of medication safety.
Communication: Ensures timely and consistent communication.
Quality: Checks and controls quality and accuracy of work performed by pharmacy team members.
Teamwork: Develops and delivers effective education to pharmacists and other healthcare providers.
The incumbent may be asked to perform additional duties as assigned. QUALIFICATIONS Education: 4 Year/Bachelor's Degree required - Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification. Experience: 1 year of experience required - Postgraduate year 1 residency or fellowship OR a minimum of 2 years of experience as a registered pharmacist in health system pharmacy.
