Job Summary

Under minimum supervision, performs one-on-one patient centered needs assessment, care coordination, and behavioral health education, including psychosocial support. Primary duties include facilitating individualized behavior change interventions to meet patient needs using motivational interviewing. The Behavioral Health Educator will supervise clinic-wide screening of patients for substance use and addictions, depression, anxiety, and overall psychosocial functioning, promote staff engagement, and provide referrals to appropriate community resources. The Behavioral Health Educator provides Brief Intervention or Referral to Treatment for patients at-risk for chemical dependency. Other duties include collaborating with hospital wide health education initiatives as needed, identifying new and existing resources and organizations that serve patient health needs, and maintaining community partnerships.

Essential Functions:

  • Develop and deliver educational programs on a variety of behavioral health topics to individuals, families, and groups (20%)
  • Collaborate with interdisciplinary teams to develop and implement behavioral health education initiatives (20%)
  • Provide support and resources to individuals and families who are impacted by behavioral health issues (10%)
  • Conduct assessments to identify training needs and develop training plans (10%)
  • Develop and distribute educational materials, including brochures, handouts, and online resources (10%)
  • Maintain accurate and timely documentation of training and education activities (10%)
  • Participate in community outreach activities to raise awareness of behavioral health issues (10%)
  • Participate in quality improvement initiatives and peer review processes (10%)

Education:

  • Bachelor's Degree in public health, human services, health sciences, psychology, sociology or related field. Required

Work Experience:

  • 1-3 years Three years health care or health education experience

Licenses:

    Knowledge, Skills and Abilities:

    • Knowledge of comprehensive health education and substance use, and experience with behavior change counseling.
    • Ability to coordinate health education within the clinic and also with community resources.
    • Proficiency in MS Office applications; experience with Excel, Word, and PowerPoint.
    • Experience facilitating one-on-one confidential visits and managing case loads.
    • Demonstrated ability as a self-starter who can prioritize and conduct multiple activities.
    • Strong communication and interpersonal skills; a team player.
    • Exceptional organizational skills, excellent oral and written communication skills and creative problem solving abilities.
