Safe Harbor/Medical Respite Case Manager

Position Summary:

Assist clients in the coordination of care to gain access to treatment services and promote the adoption of behaviors to maintain and improve individual and community health for the clients of Safe Harbor and Medical Respite.

Essential Responsibilities:

Serve as case manager to Safe Harbor and Medical Respite clients.



  • Assist clients in determining the steps needed to take to gain access to treatment services and support the client with identifying and adequately addressing barriers.
  • Assess social needs of program participants and provide resources to the extent possible for the client to utilize when in the community.
  • Direct, advocate and support clients with gaining prior authorization and coordination of treatment and integrated health services.
  • Advises clients individually or in group on issues related to risk prevention, diagnostic screenings, and community resources that improve social determinants of health outcomes.
  • Advises clients on issues related to improving general health such as diet or exercise.
  • Maintain case records in accordance with Harbor Light standards.
  • Assist with transporting clients to scheduled health appointments or referral sites, as needed.
  • Is responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
  • Participate in outreach activities with outside referral resources and community agencies to promote education on program and to increase utilization of program census.
  • Completes required authorization processes and documentation as required for medical respite program participation
  • Document and report on program outcomes to ensure outcome standards are met
  • Creates and promotes adherence to the documented care plan, developed in coordination with the patient, primary care provider(s) and/or behavioral health provider(s)
  • Attends all staff meetings and team conferences.
  • Participates in educational courses, seminars, workshops, current literature, and in-service meetings to enhance treatment skills.
  • Establishes and maintains a nurturing and supportive environment.
  • Establish and maintain professional and timely documentation in the electronic medical records system. The case manager is responsible for his/her own documentation and is expected to review, correct, and seek assistance to ensure compliance.
  • Perform other duties as assigned.


Qualifications:

Education/Experience:



  • A bachelor's degree in social work or an equivalent comparable human services field is required.
  • Experience in case management preferred.


Skills, Knowledge & Abilities:



  • Ability to effectively present information one-on-one, with co-workers, and groups
  • Knowledge and skill sufficient to use community resources necessary for independent living.
  • Must have ability to understand and carry out written, verbal, and/or demonstrated instructions.
  • Able to work alone and as part of a team.


Computer Skills:



  • Proficient personal computer skills including e-mail, internet, word processing, spreadsheet, etc.


Certificates and Licenses:



  • Valid Michigan Chauffeur License and approved by The Salvation Army MVR
  • Community Health Worker certified within 6-months of hire.
  • Complete Safe from Harm training, and keep current as needed
