Job Overview:

This position is a Internal Medicine Clinical Pharmacist with a focus on antibicrobial stewardship. This individual is responsible for optimizing clinical outcomes of patients in the inpatient medical/surgical units and related programs, and promote appropriate medication utilization in those patients. This individual will also be responsible To optimize clinical outcomes of patients and promote appropriate antimicrobial utilization, while minimizing unintended consequences of antibiotic therapy and reducing costs, by using prospective monitoring, prescriber feedback, and guideline implementation, in addition to other tools

Qualifications:

Required:

  • BA/BS in Pharmacy
  • NJ licensure
  • One year pharmacy experience, preferably in hospital, with residency or comparable experience required

Preferred:

  • Specialized training or experience in antibiotic stewardship and/or internal medicine, an
  • Previous hospital experience

Scheduling Requirements:

  • Full-Time
  • Day Shift
  • 40 hours per week

Essential Functions:

  • Direct patient care monitoring to assure appropriateness of medication therapy
  • Responsible for reporting identified safety issues such as hazardous environments, adverse drug reactions, medications, and medical errors, and daily activities.
  • The pharmacist will function as the co-chair of the antimicrobial stewardship committee and work collaborative with the physician co-chair.
  • Consult, and Educate pharmacy and nursing staff on best (medication) practices
  • Identify, promote, and implement safe medication practices
  • Assure medication related practices are being done in accordance with written policy
  • Track clinical interventions, assemble and evaluate Department documentation metrics
  • Monitor the medication reconciliation process
  • Develop the clinical skills of the staff pharmacist and maintain staff performance and consistency with clinical cost containment tasks
  • Precept pharmacy students and/or Residents when applicable
  • Communicate, and present clinical data and Department metrics at various committees
  • Perform other duties as directed by department management
