JOB SUMMARY The Mental Health Specialist/Case Manager IV reports to the MHS Supervisor and is responsible for providing specialized services to clients in gaining access to needed medical, social, housing, economic, educational, and other direct services.
ESSENTIAL JOB RESPONSIBILITIES
- Provide mental health services to children, youth and care providers in settings such as client residence, school, community locations, tele-health and clinic sites.
- Provide direct services, including case management and rehabilitation skills to clients. Such services may include group counseling, individual,and/orother forms of direct service as outlined in the Plan of Care.
- Collaborate with community partners as part of mental health activities.
- Maintain appropriate case recording documentation as per the Rehab option Medi-Cal guidelines, case recording, and other contract guidelines.
- Maintain andsubmitall documentation and billing as it relates to client services ina timelyfashion.
- Meet monthly key performance metrics, including end of the month deadlines, productivity standards and documentation timeliness expectations.
- Employees shall follow the reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse and Tarasoff warnings per California reporting laws.
- Develop andmaintainoutside supportive referral services for clients.
- Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients.
- Attend job-related trainings and participate in a variety of in-service training, and seminars.
- Attend weekly staff meetings.
- Think and act quickly and efficiently in emergencies.
- Complete training requirements set by the agency and county.
- Will drive on agency business, either in a company vehicle or personal car, including picking up, transporting and dropping off clients.
- Physical presence at the program is required.
- Employees are expected to manage their cell phone use so that the Agency cell phone stipend covers business usage related to two-factor authentication (2FA) and required HRIS access.
ESSENTIAL JOB REQUIREMENTS
- Bachelor's degree in social work or counseling in a related field.
- One (1) year experience working in a similar or related field (preferred).
- Communicate effectively in written and spoken English.
- Computer skills in Office 365, Teams, Zoom and other communication/meeting platforms.
- Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
- Ability to pass a pre-employment criminal background check.
- Ability to pass a pre-employment drug screen and TB test.
ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL
- Communicate effectively in written and spoken Spanish.
- Required ability to communicate effectively with monolingual Spanish speaking clients andfamiliesfor all clinical purposes.
- Assistwith phone calls for Spanish speaking clients.
- Required to provide language translations for case managers and Physicians on a periodic basis.
- Minimal adequate competency in the Spanish language asevidencedby successful passage of the TPOCC Spanish Competency Test.
- Other translation tasks asrequired.
