Develop and implementindividualized treatment plansin collaboration with patients and care teams. Deliveroutpatient psychotherapy services Engage patients' socialsupportsto improve functioning and outcomes. Coordinate care with community providers and agencies; initiate referrals as needed. Develop and maintain individualized treatment and care plans in collaboration with patients and families. Facilitate individual, group, and family therapy sessions aligned with clinical goals. Coordinate care with other providers and agencies; initiate referrals asappropriate. Ensureaccuratedocumentation and compliance with CMS, MN DHS, and MN 245I requirements. Participate in clinical supervision for Mental Health Practitioners when applicable. Required Qualifications* Doctoral/Ph.D. level advanced degree in Psychology or Counseling OR Masters in Social Work MN licensure asLicensed PsychologistorLICSWupon hire. Your starting rate will depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).
