The final rate offered will be determined by the collective bargaining agreement, internal salary equity, candidate experience and qualifications, and budget constraints. It is not typical for an individual to be offered a rate at or near the top of the full range for a position. This position requires Illinois residency within 180 days of the hire date. Position Summary The Clinical Nurse Consultant II (CNC II) for Cancer Prevention and Genetics is accountable for facilitating the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The CNC II provides patient care and consultation, coordinates the educational development of staff members, and acts as a role model in clinical practice. Through shared leadership, Evidence-Based Practice, Relationship-Based Care, and standards of nursing excellence, the CNC II works to improve patient outcomes while supporting unit, clinic, and organizational goals. The CNC II serves as a clinical expert, educator, consultant, and change agent for cancer prevention and genetics across primary care clinics. The CNC II collaborates with primary care providers, nurses, care managers, genetic counselors, specialists, and other members of the multidisciplinary team to identify patients at increased hereditary cancer risk, facilitate appropriate genetic evaluation and testing, promote risk-appropriate cancer screening, and support patients and families in understanding and managing hereditary cancer risk. A key responsibility of this position is the implementation, education, optimization, and sustainability of NEST Genomics, which integrates clinical decision support and care management into primary care workflows, making family cancer history actionable and supporting timely identification, evaluation, and management of patients at increased hereditary cancer risk.
