Palliative Care Nurse Practitioner/CNS $5,000 Sign on Bonus

Assured Hospice (Part of Optum & LHC Group)

This role provides specialized palliative care services to patients with serious illnesses who are not yet ready for or eligible for hospice care. The clinician partners with patients, families, and caregivers to manage symptoms, improve quality of life, and develop individualized care plans.

Key Responsibilities

  • Conduct comprehensive palliative care assessments, including physical, emotional, psychosocial, and spiritual needs.
  • Review medical history, coordinate care, and perform medication reconciliation.
  • Develop and implement evidence-based treatment plans for patients with serious illnesses.
  • Provide education and support to patients, families, and caregivers.
  • Collaborate with interdisciplinary teams to ensure high-quality patient-centered care.

Required Qualifications

  • Active RN/APRN license in the state of practice.
  • Nurse Practitioner or Clinical Nurse Specialist with prescriptive authority.
  • Current BCLS and CPR certification.
  • 5+ years of clinical nursing experience.
  • Valid driver's license and reliable transportation.

Preferred Qualifications

  • 1+ years of experience as an APRN in Hospice, Palliative Care, or Oncology.
  • Advanced Certified Hospice and Palliative Nurse (ACHPN) certification or willingness to obtain when eligible.

Compensation for this specialty generally ranges from $100,722 to $151,084. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
