The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. As a Patient Navigator, you'll help families navigate the process of obtaining an Initial Developmental Evaluation and connect them with appropriate therapies and services along the way. From completing patient intakes and coordinating screenings to following up with families and tracking outcomes, you'll help make the path to care clearer and more manageable. To be considered for this role, you must have a minimum of one year of experience in early childhood systems of care or other disability-related activities. Candidates should have knowledge of early childhood developmental diagnoses, including speech delay, autism, and global developmental delay. While a bachelor's degree is preferred, strong communication, organizational, and time-management skills are needed, along with the ability to work effectively with a variety of organizations and personnel with compassion, tact, and clarity. Familiarity with Microsoft Excel and the ability to learn and use Electronic Medical Record systems are also important. Spanish-English bilingual skills are strongly preferred.
