JOB TYPE Classification: Nonexempt Supervises Positions: JOB SUMMARYThe Masters of Social Work/Licensed Clinical Social Work (MSW/LCSW) Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans. Collaborates with RN Case Manager's for guidance with medically complex cases. Handles complex cases requiring advanced social work knowledge. The MSW/LCSW Case Manager is also responsible for facilitating coordination of care. Provides guidance and shares expertise with other case managers, leads, and conducts training sessions on new resources and programs. The MSW/LCSW Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care This shall be in accordance with the current policies and procedures, mission, vision, and values of UVACH. Serves as a resource for discharge planning and care coordination for psychosocially complex cases. Maintains current knowledge of available community resources and post-acute care options. QUALIFICATIONS Education:Graduate Degree required - Master's Degree in Social Work from school accredited by the Council on Social Work Education. Experience:2 or more years of professional social work experience or 2 or more years of healthcare experience preferred. Case Management or related experience preferred. Licensure:Licensed Clinical Social Worker (LCSW) Certification required. Additional Skills/Requirements Required:Weekend and holiday coverage. Additional Skills/Requirements Preferred:Knowledge/Competence of Microsoft Office products and keyboarding. Competencies:

PHYSICAL DEMANDS Physical Demand Code:Sedentary to Light Physical Demand: The job requires frequent sitting, standing, walking, and bending/stooping. Frequent repetitive body movements. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull up to 20 lbs. occasionally. The base compensation range for this role is $29.83 - $48.44 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education. Please note that the pay range does not include any applicable incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for those in eligible positions.
