Clinical Social Worker II/III- East Campus - 140785

Under general supervision, and in addition to performing the full range of operational level duties, responsible for administering social service programs or relevant work. Provides social work services consisting of discharge planning, continuing care services, and clinical social work services, including crisis intervention, mandated reporting, information and referral, and coordination of health and mental health services with community providers.

The Clinical Social Worker is responsible for working on the Senior Behavioral Health Unit, Medical Floors, and the Emergency Department, depending on staffing needs. Providing clinical support to the patient population with individual and support system interventions, resource education and access, unsheltered, poly substance abuse/dependence, complex care management, grief, end of life, change in functioning levels, discharge planning, crisis intervention, and care management planning. Weekends, holidays and covering for vacations & sick time.

• A Master's degree in social work from an accredited school.

• At least two (2) years of post-MSW experience. To be considered for the Clinical Social Worker III, the candidate must have at least three (3) years of post-MSW experience and be a Licensed Clinical Social Worker (LCSW).

• Must have proven independent thinking, decision making, problem-solving and prioritizing skills.

• Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients, and external constituents, both verbally and in writing.

• Bilingual in both English and Spanish.
• Epic experience.
• Two (2) years of post-graduate clinical experience.
• Two (2) years of experience working with varied levels of population.
• One (1) year of experience working in a hospital or residential setting.
• Experience working with high levels of social determinants patient populations, polysubstance abuse/dependence patient populations, unsheltered, and medical social work.

• Must be able to work various hours and locations based on business needs.
• Employment is subject to a criminal background check and pre-employment physical.
• Shifts will be rotating, 5 days a week which may include weekend shifts and weekend rotations.
