About this Opportunity At UVA Continuum Home Health Care, you will use your skills as a physical therapist to provide care to newborn and pediatric clients in their homes. We offer direct clinical care PT jobs, providing high-level quality patient care in the community. As a home healthcare clinician, you are responsible for assessing, identifying, planning, and providing care with each patient in the home environment. What can you expect in this role?

We are an academic home health agency, providing care in the community for almost 30 years. We serve Central Virginia, including Charlottesville, Albemarle, Augusta, Rockingham, Harrisonburg, Staunton, Waynesboro, Nelson, Buckingham, Fluvanna, Louisa, Orange, Madison, Greene and Culpeper.
