Description
Summary: The Program Manager ACO and CIN is responsible for managing relationships with independent partners in the ACO and CIN network, ensuring the growth, health, and sustainability of the network of clinicians in the Accountable Care Organization (ACO) and Clinically Integrated Network (CIN). This role involves coordinating network integration, managing operations, and facilitating engagement with ACO and CIN participants. Responsibilities:

Network Management
- Take full responsibility for the growth, health, and sustainability of the network of clinicians in the Accountable Care Organization (ACO) and Clinically Integrated Network (CIN).
- Develop relationships with practice managers and clinicians to help coordinate network integration.
- Facilitate engagement with ACO and CIN participants through management of needs/requests.
- Distribute all material to participating providers through Portal/email depending on participants' needs.
- Upkeep an accurate clinician roster and provide roster updates for all ACO and CIN requirements.
- Distribute clinician roster on a recurring basis to CMS and all appropriate payers.
- Collaborate with different vendors/internal teams that require ACO and CIN roster information distributed in a specific manner.
- Manage all operations of HealthTrio portal, including uploading reports, schedules, educational materials, and other documents needed at the practice level.
- Ensure independent partners are registered on the HealthTrio portal.
- Manage an accurate contact list for all practices involved in the ACO and CIN.
- Commit to developing new ways for the ACO and CIN network to grow and foster a positive and engaging environment for participating clinicians.
- Work with Provider Relations Managers to ensure an effective workflow is maintained for building and maintaining relationships with ACO and CIN partners.
- Work with various teams to manage projects/implementations related to improving services provided to the ACO and CIN network.
- Contact independent partners and communicate all network activities.
ACO Recruitment
- Plan, schedule, and prepare materials for all ACO recruitment activities for Network Leadership.
- Strategically plan all recruitment travel to target potential ACO groups.
- Create, customize, print, and prepare all materials for ACO leadership to present during ACO pitch meetings.
- Utilize multiple data sources to gather key information on target recruits. Information includes added value to ACO, quality scores, financial performance, previous ACO performance history.
- Maintain a centralized tracking system for all recruitment activities, timelines, and outcomes.
- Collaborate with Provider Relations Managers to discuss and plan visits for ACO recruit targets.
- Provide follow-up communication for ACO recruits to initiate the contracting process as well as provide additional insight to any questions or concerns developed after meeting with ACO leadership.
- Conduct onboarding meetings with new ACO participating providers to establish communication expectations, quality reporting, and the support we will provide.
Event Coordinator
- Schedule and coordinate meetings for a broad variety of ACO and CIN efforts.
- Create an annual plan for meeting cadence of ACO and CIN meetings.
- Track attendance for all participants in various meetings.
- Responsible for sending out all invites to network partners for ACO and CIN activities.
- Upkeep internal huddle meeting for ACO and CIN partners.
- Coordinate all in-person and virtual meetings with independent partners.
- Coordinate all in-person events with independent partners in all ministries.
- Plan logistics and details for all in-person events that require decor, food, drinks, etc.
- Schedule meetings with various key players in ACO and CIN activities.
- Coordinate all ACO and CIN meetings with aligned partners, which includes:
- Development of the slide deck for each meeting.
- Selection of meeting times and ensuring all elements of a proper meeting are working properly (link, microphone, cameras, etc.).
- Help coordinate the distribution of information between payers and the independent networks of the ACO and CIN.
- Send reports, contracts, presentations, and other essential documents directly to practices.
- Coordinate with Providers Relation Managers to distribute material to ACO and CIN Network.
Content
- Generate all content regarding ACO and CIN activities.
- Create PowerPoints for ACO educational events to be presented at virtual/in-person meetings.
- Develop content related to ACO and CIN activities for external distribution.
- Work with internal marketing team to enhance marketing materials.
- Develop slide templates for all ACO and CIN activities, including all education opportunities, recruitment material, and various other content.
- Create and manage slides required by ACO and CIN team for various content of the ACO.
- Coordinate with internal teams to develop ACO and CIN content.
- Generate all slides for ACO and CIN recruitment activities, coordinating with marketing.
- Create supporting documentation in collaboration with marketing team to distribute during ACO recruitment season.
- Create PowerPoints for CIN content used in virtual/in-person meetings.
- Develop onboarding content and materials to present to groups who are new to the CHQCA.
- Develop handouts used for education purposes regarding ACO and CIN activities.
- Make slides regarding ACO and CIN strategy, current performance, and future goals for internal communication purposes.
- Generate all slides for ACO Board activities, including distribution policy.
Financials
- Manage all ACO and CIN finances, making sure accurate records are kept for all cash flow activities.
- Responsible for reporting accurate amounts for all Network financial activities.
- Calculate all care management fees, quality bonuses, shared savings, and other funds received for various Population Health efforts.
- Initiate all payment distribution for all ACO and CIN funds, ensuring checks are direct deposited/sent to the correct location.
- Ensure accurate records are kept for ACO and CIN payments to key stakeholders.
- Provide documentation for all payments made to ACO and CIN stakeholders.
- Coordinate with independent partners to ensure Infor accounts are set up correctly.
- Reporting all ACO and CIN financial activities to various stakeholders at CHRISTUS Health.
- Create receipts for all independent payments through a mail merge process.
- Facilitate all communication of payment activities to ACO and CIN network, answering all questions/complaints about payment information.
- Coordinate with the Finance team at CHRISTUS to ensure accurate records are kept for booking purposes.
- Ensure calculations for payments to providers are accurate and visible to relevant parties.
Other
- Possesses strong provider-facing skills, which include effective communication, relationship management, and the ability to understand and address provider needs and concerns.
- Adept at using Microsoft Suite applications and open to learning various new systems.
- Responsible for additional duties related to the role as assigned.
Job Requirements: Education/Skills
- Bachelor's degree in health administration, business administration, or a related discipline is required
Experience
- 3 - 5 years of healthcare experience preferred
- Proficiency in healthcare systems and familiarity with policies and procedures are required
Licenses, Registrations, or Certifications
In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all Associates are required to obtain the required certifications for their respective positions within the designated time frame. Work Schedule: 8AM - 5PM Monday-Friday Work Type: Full Time
