JOB SUMMARY

The ACT/FACT Education and Employment Mental Health Specialist/Case Manager V reports to the Supervising Personal Service Coordinator works as a part of the ACT/FACT treatment team in achieving the goals and objectives of the program to ensure quality of care for client participants. The MHS is responsible for assisting clients in obtaining desired educational goals through access to vocational training schools, community and local colleges, universities and internet-based programs. The MHS will also provide assistance with the client's individual needs for job development, job seeking skills, job retention skills, on-the-job assessment, training, and support.

ACT/FACT is a community-based, team-based service to support members living with complex and significant behavioral health needs and a treatment history that may include psychiatric hospitalization and emergency room visits, residential treatment, involvement with the criminal justice system, homelessness, and/or lack of engagement with traditional outpatient services.



ESSENTIAL JOB RESPONSIBILITIES



  1. Under the supervision of the Supervising Personal Service Coordinator, assist in the accomplishment of the case management plan with clients by screening, evaluating, identifying, and implementing specific case management activities as identified in the treatment plan to support clients in achieving their goals.
  2. Plan, coordinate, and facilitate services for Severely Mental Ill (SMI) clients.
  3. Work on a multi-disciplinary team to support clients with an array of needs delivered as an integrated service, including, but not limited to, medications, therapy, and groups, housing, employment, benefits, transportation, physical health, and substance use.
  4. Provide frequent and assertive contact to individuals, primarily community/ field-based services with occasional office-based services.
  5. Provide direct services, including case management and rehabilitation skills to clients. Such services may include group counseling, individual, and/or other forms of direct service as outlined in the Plan of Care.
  6. Provide orientation, career planning, and employment education development services to clients.
  7. Provide individual and group guidance to clients assisting them in the development of skills necessary to maintain employment and continue educational goals.
  8. Assess clients for work readiness.
  9. Develop individual education/employment or training plans with clients.
  10. Provide classroom educational instruction or training plans with clients.
  11. Develop employment and or work experience opportunities in the local community for clients.
  12. Develop and maintain outside supportive referral services for clients.
  13. Provide counseling to clients to assist in development of job relation skills.
  14. Attend daily team meetings to consult and coordinate client care.
  15. Be available to respond to crisis in the community.
  16. Attend weekly supervision meetings with the Supervising Personal Service Coordinator.
  17. Maintain and submit all documentation and billing as it relates to client services in a timely fashion.
  18. Meet monthly key performance metrics, including end of the month deadlines, productivity standards and documentation timeliness expectations.
  19. Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients.
  20. Provide the Supervising Personal Coordinator with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.
  21. Think and act quickly and efficiently in emergencies.
  22. Complete training requirements set by the agency and county.
  23. Will drive on agency business, either in a company vehicle or personal car, including picking up, transporting and dropping off clients.
  24. Physical presence at the program is required.

Qualifications

ESSENTIAL JOB REQUIREMENTS



  1. A bachelor's degree from an accredited or regionally approved college or university in social work, psychology, vocational rehabilitation, criminal justice, or a related field that is directly related to the specific responsibilities of the job.
  2. At least one year of experience in a mental health or human services related field.
  3. Current certified training in emergency first aid procedures preferred.
  4. Knowledge of best practices regarding the treatment of clients with Severe Mental Illness (SMI) and Dual-Diagnosis individuals.
  5. Communicate effectively in written and spoken English.
  6. Intermediate computer skills in Office 365, Teams, Zoom and other communication/meeting platforms.
  7. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  8. Ability to pass a pre-employment criminal background check.
  9. Ability to pass a pre-employment drug screen, and TB test.



ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL



  1. Communicate effectively in written and spoken Spanish.
  2. Required ability to communicate effectively with monolingual Spanish speaking clients and families for all clinical purposes.
  3. Assist with phone calls for Spanish speaking clients.
  4. Required to provide language translations for case managers and Physicians on a periodic basis.
  5. Minimal adequate competency in the Spanish language as evidenced by successful passage of the TPOCC Spanish Competency Test.
  6. Other translation tasks as required
