About the Job

Objective/Overview

CUHCC's mission is to seek health equity in our community by advancing the well-being of diverse people. This position provides mental health therapy and assessment services to patients in an outpatient setting as part of the mental health outpatient therapy team. The position has independent responsibility and accountability in clinical decision-making and performing the duties of an independently licensed mental health professional as defined by MN statute.

A qualified candidate for this position will be hired into one of three job codes depending on licensure. With the exception of licensure, essential functions and minimum requirements are the same across all three job codes. The job codes and licensure requirements are as follows:

  • Licensed Professional Clinical Counselor (LPCC), and be placed in a 9721CG (HC Pro 5 Counseling) job code
  • Licensed Independent Clinical Social Worker (LICSW) and be placed in a 9721SW (HC Pro 5 Social Work) job code or
  • Licensed Marriage & Family Therapist (LMFT) and be placed in a 9721MF (HC Pro 5 Marriage & Family Therapy) job code

Please note, this position is not eligible for H-1B or Green Card sponsorship. This position does not offer a STEM OPT training program.

Reporting Relationship: This position reports to the Therapy Program Manager.

About the Position

Primary work environment is on-site at the CUHCC clinic during standard clinic hours (typically 8:00 AM - 5:00 PM, with slight variations), including at least one dedicated telehealth day. There is flexibility for later hours on telehealth days if preferred. The final schedule is designed to balance patient needs with productivity expectations. This role may come into contact with patients who may have contagious illnesses.

Job Responsibilities

Therapy and Assessment Services (70%)

Assessments: Conduct reflective diagnostic interviews and establish accurate clinical diagnoses within a distinct cultural context. Utilize standardized assessment screeners (e.g., PHQ-9, GAD-7, PC-PTSD, CAGE-AID) to evaluate initial baseline needs and monitor ongoing patient response to care using established clinical protocols. Additionally, participate as requested in completing supplementary patient documentation, including medical opinion forms, diagnostic verifications, and immigration or victim support letters, as an integral part of the assessment process.

Treatment Planning: Actively engage in patient treatment planning by developing and updating individualized treatment plans in strict accordance with DHS guidelines and clinical timelines. Formulate tailored, comprehensive case conceptualizations specifically designed for a complex trauma population.

Outpatient Therapy: Deliver responsive individual, group, couple, and family therapy interventions that provide trauma-informed, culturally competent care tailored to the population's complex needs. Engage in ongoing professional development to continually adapt these interventions to evolving patient demographics and clinical needs. To support overall clinical excellence and patient retention, actively integrate patient feedback to improve services while intentionally developing trust, safety, and strong therapeutic rapport.

Productivity: Maintain a regular volume of 28+ scheduled encounters per week to meet a 1.0 FTE baseline target of 982 billable encounters per fiscal year, adjusting volume to offset planned time off, sick leave and clinic closures.

Documentation & Electronic Health Record Management (10%)

Complete mandated clinical documentation in the electronic health record (Epic Ochin) using designated clinical templates and workflows. Maintain a high 48-hour note signature rate, minimize backlogs past 7 days, and ensure accurate CPT coding according to clinic policy. Ensure all treatment plan updates are completed in a timely manner every 180 days as required by DHS guidelines. To optimize efficiency and balance workload throughout the clinical day, utilize collaborative or concurrent documentation strategies. Staff are encouraged to remain flexible and open to utilizing advanced documentation technologies and AI note supports, while actively partnering with the clinic's EHR support analyst to address workflows, support needs, or technical issues.

Integrated Care & Team Consultation (10%)

Actively participate in therapy team meetings, medical and mental health combined consultations, and interdisciplinary care across all clinic service lines. Build and maintain strong professional relationships with colleagues in medical, psychiatry, and community programs to ensure comprehensive, whole-person care. Coordinate closely with the patient care team by sharing clinical feedback and maintaining high responsiveness to Epic Ochin patient care messages. Additionally, collaborate across departments to provide crisis consultation and clinical support as needed to assist other teams in managing patient crisis or acute mental health needs throughout the clinic.

Professional Development & Quality Improvement (10%)

Participate in all required clinic training and maintain continuing education requirements as mandated by the licensing board. Actively contribute to departmental excellence by engaging in required quarterly peer reviews and participating in mental health department quality improvement projects. Additionally, support the clinic's training mission by fostering a rich learning environment for the mental health department. Demonstrate flexibility and adaptability by opening clinical sessions for student shadowing, providing vital mentorship, and facilitating reflective processing to guide and develop learners.

***This position requires a negative TB test, and a Hepatitis B vaccination.

Qualifications

Required Qualifications

  • Completed a Masters or Doctorate degree program in counseling, social work, marriage & family therapy or a related field, plus at least four years of experience, or a terminal degree plus at least one year of experience.
  • Independently licensed Mental health Professional as defined by MN statute either as a Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage & Family Therapist (LMFT) or a Licensed Professional Clinical Counselor (LPCC). License must be active and in good standing.
  • Meet mandated annual requirements for continuing education per the professional's licensing board.

Preferred Qualifications

  • Experience in providing clinical services to immigrant, refugee or other underserved populations in a multicultural practice setting.
  • Experience working within an integrated care model or Federally Qualified Health Center (FQHC).
  • Advanced training and experience in Trauma-Informed Care and other evidence-based treatment modalities (e.g., EMDR, DBT, TF-CBT, Brainspotting) tailored to complex trauma populations.
  • Fluency in Spanish, Somali, French or other languages and/or demonstrated competence navigating complex language barriers.
  • Experience utilizing Epic/Ochin EHR systems.
  • Experience working within a teaching clinic, with a strong interest or background in providing clinical mentorship, session shadowing, and reflective processing for clinical trainees and/or be eligible to provide board-approved clinical supervision.

Pay and Benefits

Pay Range: $75,000 - $85,000 depending on education/qualifications/experience
