Type of Opportunity: Full time FTE: 1.00 Job Exempt: No Work Shift: 10 Hour Days (United States of America)

Responsibilities:

  • Oversees patient care delivery by directing and delegating assignments based on acuity, staff capability, and anticipated needs.
  • Provides clinically competent direct care, evaluates outcomes, consults with specialists, and adjusts nursing processes to ensure optimal patient care.
  • Coordinates patient care with the patient, family, interdisciplinary team, physicians, and community resources.
  • Demonstrates knowledge of care philosophy and offers support to patients and families throughout the care process.
  • Ensures patient understanding of rights, responsibilities, informed consent, and advanced directives during admission.
  • Assesses and directs patient/family teaching using return demonstration and appropriate teaching aids in the home setting.
  • Maintains professional responsibility for current nursing research, practice standards, protocols, and organizational policies.
  • Maintains proficiency with all equipment used in home health practice.
  • Communicates effectively with nursing and interdisciplinary team members and participates in problem solving and conflict resolution.
  • Performs oncall duties including triage, phone support, and patient visits as needed.
  • Performs other functions as required

Qualifications:

Academic Preparation

  • Degree from an accredited academic nursing program (programs under conditional status may be approved by the system CNO).
  • Minimum ADN preparation.

Licensure

  • State of New Mexico or Compact State Nursing License required.

Certifications Required certification from AHA, ASHI, or Red Cross:

  • BLS: All RN roles
  • ACLS: Emergency Department, Critical Care, Cardiovascular Lab, GI Lab, Interventional Radiology, Progressive Care, Surgical Services PreOp and Recovery Room
  • NRP: Family Birthing Center, Labor and Delivery, Neonatal Intensive Care, Mother/Baby Care
  • PALS: Children Services, Cardiovascular Lab, Emergency Department, GI Lab, Interventional Radiology, Surgical Services PreOp and Recovery Room
