Position Overview

The Wound Care Registered Nurse is responsible for conducting comprehensive health assessment interviews focused on wound, ostomy, and continence-related care. This role performs focused physical assessments, develops individualized plans of care, and recommends appropriate medications and treatments based on patient diagnoses and clinical needs. The RN collaborates closely with interdisciplinary teams to support quality patient outcomes, provide patient and staff education, and implement evidence-based wound care practices. The position also supports quality improvement initiatives, promotes culturally competent care, and ensures compliance with patient confidentiality and professional nursing standards.

Job Responsibilities




  • Develop, revise, and evaluate standards of care while assisting in the selection of supplies, technology, and equipment.



  • Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wound treatments.



  • Evaluate the quality and delivery of care provided by clinical staff within assigned units.



  • Collaborate with the interdisciplinary healthcare team and make referrals to appropriate disciplines as needed.



  • Teach, counsel, and assist patients and families in preventing complications related to wounds, ostomies, and continence issues.



  • Develop, provide, and evaluate staff education programs, including orientation, in-service training, and continuing education.



  • Conduct comprehensive assessments of patients at risk for pressure injuries and initiate preventative interventions.



  • Escalate and communicate identified challenges, including ethical concerns and conflict resolution issues.



  • Collaborate with nursing leadership to recommend and implement best practices based on clinical assessments.



  • Participate in departmental and organization-wide quality improvement initiatives, including incidence and prevalence studies.



  • Provide culturally competent education to patients, families, and staff while respecting cultural, ethnic, and religious beliefs.



  • Maintain the security and confidentiality of patient information in accordance with organizational policies and patient rights standards.



  • Maintain professional competency through ongoing clinical practice and continuing education activities.



  • Identify and coordinate community resources to support patient care and continuity of services.



  • Manage assets related to rented equipment, including specialty beds and negative pressure wound therapy devices.




Job Requirements




  • Active Registered Nurse (RN) license in the State of New York.



  • Primary Source Verification of RN license and registration through the New York State Department of Education (NYSED).



  • Current AHA BLS certification.



  • Current certification in at least one of the following:




    • Certified Wound Ostomy Continence Nurse (CWOCN)



    • Certified Wound Ostomy Nurse (CWON)



    • Certified Wound Care Nurse (CWCN)



    • Certified Wound Specialist (CWS)



    • Wound Care Certified (WCC)





  • Minimum of two (2) years of Medical/Surgical nursing experience.



  • Experience in wound and colostomy care required.



  • Strong clinical assessment, communication, documentation, and patient education skills.



  • Ability to work collaboratively within a multidisciplinary healthcare environment.



  • Knowledge of wound care standards, infection prevention practices, and quality improvement processes.




Education




  • Associate Degree in Nursing (ASN or ADN) from an accredited nursing program required.

Additional Job Details (Commute, Attendance, etc.)




  • Schedule consists of four (4) 10-hour shifts per week.



  • Every other weekend required.



  • Position is open due to vacancy.
