Provides education and emotional support to patients in the transplant department and helps guide them through the healthcare system. Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing supportive care services within BCH and in the community.
Key responsibilities:
High School Diploma / GED is required. Bachelor's Degree is preferred. Human Services or other related fields of study preferred. Experience: A minimum of one year of closely-related experience is required. Experience with the clinic population or work with community resources is preferred. The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
