*Immediate Hiring: Care Management Support Coordinator II (Entry Level)* *Job Summary* We are *immediately hiring* a *Care Management Support Coordinator II* to support administrative care management activities. This role serves as a key point of contact for members, providers, and internal staff, providing high-quality telephonic support, coordinating services, and ensuring accurate documentation in compliance with state and regulatory guidelines.
All member interactions for this role are conducted *telephonically* in a fast-paced environment. *Key Responsibilities* * Perform outbound outreach and handle inbound calls to members regarding care plan next steps, scheduling services, and available community or health plan resources * Serve as front-line support for member and provider inquiries, requests, and concerns * Explain care plan procedures, protocols, and program information to members and providers * Support members in connecting to health plan and community resources, including referrals related to *Social Determinants of Health (SDOH)* * Assist with member onboarding activities, including sending welcome letters, correspondence, and educational materials * Maintain and document *non-clinical member records* accurately, ensuring compliance with state and regulatory standards * Apply working knowledge of assigned health plan activities and resources * Provide a high level of customer service while managing multiple tasks and deadlines * Perform other duties as assigned while complying with all organizational policies and standards *Education & Experience Requirements* * High School Diploma or GED (required) * 1-2 years of related experience (required) *Preferred Skills & Qualifications* * Call center experience (inbound and outbound calling) * Strong customer service and communication skills * Comfortable working in a *fast-paced, deadline-driven environment* * Basic computer proficiency, including *Outlook and Excel* * Ability to manage high call volumes throughout the day * Bilingual skills are a plus, but *not required* *Job Type & Location*This is a Contract position based out of McAllen, TX. *Pay and Benefits*The pay range for this position is $18.50 - $18.50/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
