Our Westminster, MD office is hiring a Program Coordinator to ensure quality services and individual centered approaches forthe individuals in Alternative Living Units or community Supported Living Arrangements.

Responsibilities:

  • Develop and implement the Individual Plan
  • Assessment, planning, implementation, and review of person-centered support plans for individuals receiving services
  • Coordinate the delivery of all services provided by Benchmark Human Services and the community
  • Assist individuals served in expanding their opportunities to participate in community life as they choose
  • Provide on-call direct support and coverage coordination
  • Participate in the hiring, training and supervision of Managers and Direct Support Professionals
  • Schedule staff in accordance with IP and budget
  • Accurate and timely implementation of Individual Plans, required reporting to regional offices, individual, QA walk through, monthly and daily documentation (logs, red, yellow and orange books, fire and tornado drills, individual medical information, MARs, etc.)
  • To be aware of and submit reportable incidents within required time frames
  • Manage and monitor client finances within required time frames
  • Serve as liaison with external resource coordinators, families, other agencies and internal programs
  • Other duties as assigned

Requirements:

  • High School Diploma or Equivalency required, Degree preferred.
  • 5 years' Experience working with individuals with developmental disabilities in a residential setting.
  • Supervisory experience
  • Valid driver's license and auto insurance.
