Job Summary

The Manager of Population Health supports quality and care coordination for the UNC Health Pardee BlueMD primary care network.

Responsibilities:
* Recruit, hire, orient and lead a team of clinical professionals in collaboration with BlueMD leadership. Maintain daily operations of clinical procedures within a defined budget.
* Provides day-to-day operations supervision of population health department staff.
* Ensures the implementation of best practices in patient care and staff training to promote health and wellness.
* Monitor and evaluate the performance and productivity of assigned staff, facilitating positive patient experience and care outcomes. Meets regularly with staff members to discuss department goals and progress towards those goals.
* Analyzes population health data from the ACO, UNC and other sources to identify trends in and assist with developing strategies to improve performance in those areas.
* Collaborates in strategic planning to improve performance in all areas of Population Health department work.
* In collaboration with their leaders, responsible for managing the process of hiring new employees to fill positions within the department.

Required

* Must be licensed to practice as a Registered Nurse in the state of North Carolina
or one of compact states
* Experience performing in a leadership capacity
* Five (5) years of experience in Population Health and/or Care Management
* Strong verbal and written communication
Preferred
* Bachelor's of Science in Nursing (BSN)
* Experience in statistical analysis
* Advanced experience using or managing a Electronic Medical Recordkeeping
(EMR) system such as EPIC

Qualifications

Required


  • Must be licensed to practice as a Registered Nurse in the state of North Carolina or one of compact states

  • Experience performing in a leadership capacity

  • Five (5) years of experience in Population Health and/or Care Management

  • Strong verbal and written communication


Preferred


  • Bachelor's of Science in Nursing (BSN)
