Central Intake Specialist - Hospice - 40 Hours - Day

Shift: Days 8:30 am to 5:00 pm

General Summary:

Under general supervision of the Leader of Central Intake, is responsible for

• Intake of new referrals by phone, fax and/or computer.
• Enters referral demographics into computer,
• Coordinates insurance coverage with the Billing department and assists with coordination of care.
• Routinely interacts with physicians, nurses, case managers and the general public regarding the coordination of patients' transitions of care and ongoing care in the home.
