Position: Lead Service Facilitator / Care Coordinator Lead With Purpose. Coordinate With Impact. Professional Services Group (PSG) is seeking a dedicated and experienced Lead Service Facilitator / Care Coordinator to support adults enrolled in the Milwaukee Adult Comprehensive Community Services (CCS) program. This leadership-level role combines direct care coordination with mentorship, quality assurance, and team support responsibilities. About the Program Milwaukee Adult CCS provides community-based, recovery-focused services to individuals living with significant mental health and substance use diagnoses. Services follow the wraparound model and emphasize strength-based, person-centered care delivered in homes, community settings, and clinical environments. Position Overview As a Lead Service Facilitator, you will maintain an active caseload while also supporting program supervisors and mentoring care coordinators. You will play a key role in quality assurance, staff development, and ensuring compliance with DHS 36 and Medical Assistance (MA) regulations. Key Responsibilities Leadership and Team Support * Provide support, guidance, and problem resolution to care coordinators Direct Client Care * Maintain a caseload of approximately 12-14 clients Documentation and Compliance * Enter and maintain accurate documentation in required databases (including Avatar) Collaboration * Work closely with Milwaukee County Behavioral Health Division (BHD) Qualifications Education * Bachelor's degree in Social Work, Psychology, Criminal Justice, or related human services field required Experience * At least one year of experience working with individuals with significant mental health and/or substance use diagnoses preferred Knowledge, Skills, and Abilities * Strong understanding of recovery-oriented and strength-based practices Additional Requirements * Valid driver's license, reliable vehicle, and proof of insurance Work Environment This position operates in office settings as well as in homes, courtrooms, clinics, and community locations.
