Program: Milwaukee Adult CCS Care Coordination
Location: Milwaukee, WI
Schedule: Full-Time | Primarily Monday-Friday | Some Evening Flexibility Required
Pay Status: Hourly

Professional Services Group (PSG) is seeking a dedicated and experienced Lead Service Facilitator / Care Coordinator to support adults enrolled in the Milwaukee Adult Comprehensive Community Services (CCS) program. This leadership-level role combines direct care coordination with mentorship, quality assurance, and team support responsibilities.

About the Program

Milwaukee Adult CCS provides community-based, recovery-focused services to individuals living with significant mental health and substance use diagnoses. Services follow the wraparound model and emphasize strength-based, person-centered care delivered in homes, community settings, and clinical environments.

Position Overview

As a Lead Service Facilitator, you will maintain an active caseload while also supporting program supervisors and mentoring care coordinators. You will play a key role in quality assurance, staff development, and ensuring compliance with DHS 36 and Medical Assistance (MA) regulations.

Key Responsibilities

Leadership and Team Support

* Provide support, guidance, and problem resolution to care coordinators
* Mentor new staff and assist with onboarding and CCS process training
* Help plan and conduct team meetings
* Assist staff in developing formal and informal community resources
* Review documentation for quality assurance and compliance

Direct Client Care

* Maintain a caseload of approximately 12-14 clients
* Conduct intakes and assessments, including PPS/NOMS and Functional Screens
* Complete MA-compliant Strengths and Needs Assessments
* Develop and update Recovery Plans of Care (RPOC)
* Facilitate team meetings and monitor client progress
* Develop crisis and safety plans
* Advocate for clients in emergency and crisis situations
* Attend court proceedings, psychiatric appointments, and other supportive meetings as needed
* Administer satisfaction surveys and track outcome data

Documentation and Compliance

* Enter and maintain accurate documentation in required databases (including Avatar)
* Meet monthly productivity expectations
* Complete progress notes, assessments, service authorizations, quarterly reports, recertifications, and discharge summaries within required timelines
* Comply with DHS 36, CCS, MA, HIPAA, and agency policies

Collaboration

* Work closely with Milwaukee County Behavioral Health Division (BHD)
* Collaborate with mental health professionals, community agencies, court officials, and other providers
* Participate in weekly staffing and clinical supervision

Qualifications

Education

* Bachelor's degree in Social Work, Psychology, Criminal Justice, or related human services field required

Experience

* At least one year of experience working with individuals with significant mental health and/or substance use diagnoses preferred
* Experience in care coordination or CCS services strongly valued

Knowledge, Skills, and Abilities

* Strong understanding of recovery-oriented and strength-based practices
* Knowledge of DSM-5 diagnoses and community resources
* Ability to manage time independently while balancing leadership and caseload responsibilities
* Strong oral and written communication skills
* Ability to present cases in staffing with Milwaukee County BHD
* Proficiency in electronic documentation systems
* Ability to provide culturally responsive and inclusive services
* Sound judgment and decision-making skills

Additional Requirements

* Valid driver's license, reliable vehicle, and proof of insurance
* Ability to safely transport clients
* Ability to work in office, community, and client home environments

Work Environment

This position operates in office settings as well as in homes, courtrooms, clinics, and community locations. Exposure to varied environments, including outdoor weather and household pets, may occur.

Physical Requirements

* Ability to sit, stand, walk, and navigate stairs
* Ability to operate a computer and office equipment
* Ability to lift up to 20 pounds
* Visual acuity sufficient to review documentation and safely operate a vehicle
