The Social Worker, imbedded in adult medicine, is responsible for performing social work assessments, interventions, crisis management, and relevant casework as needed for outpatients of the adult medical department. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving in collaboration with both medical and behavioral health providers on site. As a member of a multidisciplinary health team, they perform comprehensive and culturally considerate assessments and coordinate next steps in care that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Social Workers are responsible for compassionate care of patients that supports patient-centered plan of care for both individual patients and the medical community.
Does this position require Patient Care? Yes Essential Functions -Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. -Formulates biopsychosocial assessment, disposition, and brief treatment plans. -Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. -Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan. -Responding to urgent and emergent needs of patients onsite in collaboration with medical and behavioral health team. -Providing relevant case management and care coordination relevant to behavioral and mental health needs. Education Licenses and Credentials Licensed Mental Health Counselor [LMHC - State License] - Spanish fluency required - Ability to collaborate effectively with multiple teams Physical Requirements
