The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Social Worker provides emotional and administrative support to patients, either as a group or individual, through group process and psychotherapy. All methods of practice shall conform to written hospital policies and procedures and be carried out to the highest achievable level of competence in a manner demonstrating concern and consideration. The Social Worker will demonstrate Clara Maass philosophy to Total Quality. Management by actively applying that philosophy in all dealings with both internal and external customers.

Qualifications:

Required:

  • Master s degree in social work MSW from an accredited program preferred.
  • Minimum of 2 years of experience in pediatric social work or a related field
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work effectively in a fast-paced, multidisciplinary environment.

Scheduling Requirements (non-exempt positions):

  • Per Diem
  • Weekday Days and or Evenings and Weekend and Holidays

Essential Functions:

  • Acts as a consultant for Home Health staff regarding social, emotional or behavioral problems which they are encountering with clients, with focus on how to deal with various situations. Make recommendations regarding community agencies where appropriate.
  • Assist the physician and other members of the health team in understanding significant social and emotional factors related to the client's health problems.
  • Attends at least two continuing education programs per year.
  • Conduct financial evaluations to determine billing arrangements when a client is financially unable to pay for Home Health services and does not have insurance. Evaluation follows Hill Burton guidelines. Recommendations will be subject to administrative authorization.
  • Counseling the client's family in understanding the client in his/her emotional care.
  • Employee complies with all Federal, State, local and JCAHO standards, rules and regulations.
  • Identifying the significant social and psychological factors related to the health problems of the client, assists the physician and other team members in understanding these factors.
  • Interpret community resources to Home Health staff.
  • Interpreting to the community the role of social work in the agency's program.
  • Involvement as a co-leader of groups to be offered to clients and/or family members of client.
  • Maintain appropriate records on each client.
  • Observing, recording and reporting to the physician or dentist and agency personnel, the client's and family's reaction to the treatment plan and any changes in the client's condition or family's interrelationships which may adversely affect the client's plan of care.
  • Participates in discharge planning and inservice programs as appropriate.
  • Participates in team conferences to discuss the client's current status and any related problems.
  • Participating in the planning and development of staff inservice education programs.
  • Participating with other health team personnel in the development of the plan of treatment, including discharge planning
  • Performs such other duties as may from time-to-time be assigned.
  • The employee demonstrates competence in the following: As appropriate to the ages of the clients served; the ability to obtain and interpret information in terms of the clients' needs, the clients' growth and development level and demonstrates an understanding of the range of treatment needed by those client
