Job Description: Under administrative direction, provides clinical leadership to the ACT Team in assessment, treatment planning, general healthcare, medical and psychosocial approaches. Assertive Community Treatment (ACT) is designed to engage persons with a severe mental illness (SMI) whose needs are not met by traditional outpatient services. Services are delivered in the context and environment where they are needed (i.e., the team goes to the person). AI Summary The ACT Team Lead is responsible for overseeing and coordinating the activities of an Assertive Community Treatment (ACT) team. This includes supervising team members, developing treatment plans, coordinating care for clients with severe and persistent mental illness, and ensuring that services are delivered in accordance with best practices and regulations. The ACT Team Lead also collaborates with community partners, conducts assessments, monitors progress, and advocates for clients' needs. Strong leadership, communication, and organizational skills are essential for success in this role.
