Job Description:

Under administrative direction, provides clinical leadership to the ACT Team in assessment, treatment planning, general healthcare, medical and psychosocial approaches. Assertive Community Treatment (ACT) is designed to engage persons with a severe mental illness (SMI) whose needs are not met by traditional outpatient services. Services are delivered in the context and environment where they are needed (i.e., the team goes to the person). AI Summary The ACT Team Lead is responsible for overseeing and coordinating the activities of an Assertive Community Treatment (ACT) team. This includes supervising team members, developing treatment plans, coordinating care for clients with severe and persistent mental illness, and ensuring that services are delivered in accordance with best practices and regulations. The ACT Team Lead also collaborates with community partners, conducts assessments, monitors progress, and advocates for clients' needs. Strong leadership, communication, and organizational skills are essential for success in this role.
  • Directs day-to-day clinical operations of ACT team.
    • Provides direct supervision of team members.
    • Schedules staff work hours to ensure appropriate coverage.
    • Leads daily team and treatment planning meetings.
  • Evaluates status of all consumers and provides appropriate coordination of treatment to meet their challenging needs.
  • Directs and coordinates consumer admission and assessment processes.
    • Schedules admission interview
    • Supervises, directs, and coordinates completion of comprehensive assessment/reassessment of each consumer.
  • Utilizes management tools to help ensure team's philosophy and goals are in accordance with the ACT model.
  • Participates in staff recruitment, interviewing, hiring, orientation, and performance plans.
  • Oversees medical record documentation to ensure quality and accuracy.
  • Provides on-call crisis interventions and back up according to team's policy.
  • Participates in engaging and motivating team clients in all treatment goals while providing direct service.
  • Manages ACT Team in maintaining organization's required productivity standards.
  • Oversees completion of all required documentation in a timely manner consistent with organization's guidelines.
  • Master degree in Social Work, or social services related field required
EXPERIENCE
  • Six plus years of of clinical experience. Supervisory background and experience with program management preferred
REQUIREMENTS
  • Valid Driver's license
