The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

This position will report directly to the Section Chief of Advanced Heart Failure and Vice President of Heart, Vascular, and Transplant Service Line.

Summary:

In this position, the Lead APP will exclusively work in the in-patient setting supporting our Advanced Heart Failure Cardiologist. They will also support our Advanced Heart Failure team in accordance with Joint Commission, UNOS, CMS and RWJUH guidelines, regulations and polices. Coordinates clinical and administrative process for patients through all phases of transplantation (pre, transplant and post). Educates patients, families and staff regarding transplantation. Collaborates with physicians and other program staff to evaluate and manage patients. Completes data capture, analysis and reporting as well as other administrative functions required for transplant program operations. Also participates in the care of heart failure patients.

Specific Requirements:

Clinical Practice: The APP, under the Advanced Heart Failure, Transplant & Mechanical Assist Device Service at RWJUH will practice within inpatient adult population. Pre-Transplant and Heart Failure

  • Develops and implements a collaborative practice agreement specific to the clinical areas the APP will conduct their practice within, and performs a periodic review of the collaborative agreement in accordance with hospital policy.
  • Facilitates the evaluation of heart transplant candidates beginning with the transplant evaluation consent.
  • Encourages and facilitates rapid completion of transplant evaluation and prepares case for presentation at weekly multi-disciplinary meeting.
  • Initiates and performs selected therapeutic and diagnostic procedures, consultations and laboratory examination per transplant policies.
  • Maintain fiscal responsibility in completing patient evaluation and coordinates with financial counselor for insurance clearance.
  • Maintains and ensures communication with patient, referring physicians, transplant team and other health care providers. Provides required information to UNOS, NT Sharing Network and other databases as required.
  • Maintains current medical and contact information on listed patients and updates team as needed.

Heart Failure / VAD:

Heart Failure/VAD: Participates in care of heart failure patients by performing assessments of patients including history and physical examination and well as diagnoses and treatment. Performs lab review with coordinators, acts on results and adjust treatment as needed.

  • Orders lab work, Echocardiogram, CT scans and all necessary testing needed for the care of specific patients.
  • Adheres to and implements, in collaboration with cardiologist, established written protocols for medication titration in heart failure/transplant patients.
  • Prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated.
  • Distinguishes between normal and abnormal findings and evaluates what findings need collaboration or referral to other health professionals or appropriate resources.
  • VAD: Provides direct and indirect patient care to mechanical assist device patients per Clinical Practice Guidelines for VAD patients.
  • Orders VAD equipment for community discharge on patients with private insurance.
  • Monitors and ensures proper working order of VAD equipment, collaborates with bioengineering or Thoratec as needed.
  • Submits data and shares and monitors reports from NTERMACS registry.
  • Provides leadership in the application and re-certification of Joint Commission VAD Destination Therapy Certification.

On Call:

Participates in 24-hour coverage of outpatient transplant service for donor calls and transplant coordination; responds to patient concerns in collaboration with physician as indicated.

  • Removes transplanted patients from UNOS list within 24 hours of transplant surgery and provides follow-up to NJSN coordinators in a timely manner.

Administrative Research:

Administrative, Research and Other Duties: Able to provide in-service training to various groups e.g. patients, community groups and staff.

  • Maintains current knowledge of clinical and regulatory requirements transplantation and VADs.
  • Works to obtain or maintain CCTC.
  • Reviews and shares literature to support evidence-based practice.
  • Participates in QAPI. -Provides input to clinical protocols and program guidelines.
  • Supports programmatic research activities and understands limits of participation in research if not CITI trained.
  • Promotes positive relationships with referring physicians.
  • Maintains and submits monthly time studies.
  • Performs a patient record review with the collaborating physician in a timely fashion, but not less than bi-annually.
  • A minimum of 25 charts/records will be reviewed biannually.

Qualifications:

Education:

  • Current license as a registered professional nurse in the State of New Jersey.
  • Certification by the New Jersey Board of Nursing as an Advanced Practice Nurse.
  • Master's degree in nursing required.
  • Maintains current BLS and ACLS certifications.
  • Should obtain or maintain CCTC certification.

Experience:

  • Transplant cardiology or heart failure experience required
