Job Description:

Peer Advocate Bilingual Required

  • Must be available to work onsite storefront Henderson, NV
  • Must be a current Scan Health Plan member

The Job

Support members that are at risk for specific health conditions to manage their own health. Through peer support and the relaying of shared experiences, assist to educate and motivate behavior changes to improve health outcomes and quality of life.

You Will

  • Make outbound calls to SCAN members based on identified health inventions or quality initiatives.
  • Build rapport with members by communicating personal experiences as a SCAN member and peer to enhance the effectiveness of health education and increase likelihood of positive behavior change.
  • Coach members to create and implement strategies that minimize risks associated with the identified common health conditions.
  • Encourage and provide assistance to members who may encounter obstacles to seeking care.
  • Document all required information in the care management system during phone calls with members.
  • Maintain client confidence by keeping information confidential.
  • Utilize department job aids and training materials.
  • We seek Rebels who are curious about AI and its power to transform how we operate and serve our members.
  • Actively support the achievement of SCAN's Vision and Goals.
  • Contribute to team effort by accomplishing related results as needed.
  • Other duties as assigned.

Your Qualifications

  • High School Diploma or equivalent Preferred.
  • Current SCAN Membership, required.
  • Must be proficient with operating a computer, including logging into and maneuvering around multiple systems such as online timesheets (time and attendance) and call center software.
  • Excellent communication skills including the ability to articulate one's own personal SCAN membership experience.
  • Strong interpersonal skills, as well as strong written and verbal communication skills.
  • Ability to work independently as well as part of a team.
  • Strong organizational skills.
  • Ability to work in a fast, loud and changing environment
