In Permanent Supportive Housing (PSH), we don't just offer a place to stay; we offer a foundation for a new life. As a Clinical Case Manager, you are the architect of that foundation. You'll use your advanced education to assess complex needs, determine appropriate levels of care, and collaborate with a network of agencies to ensure our members are supported, secure, and thriving.

  High-Level Assessment: You'll lead the intake and assessment process for new members, using your clinical lens to determine the specific services they need to succeed.

  Strategic Collaboration: You aren't just making phone calls; you're building a multidisciplinary network, collaborating with community agencies to ensure a seamless "whole-person" approach to care.

Schedule: Monday - Friday | 8:30am-5:00pm | Full-Time

Expected starting wage range is $26.50 - $32.50. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• Master's degree in Social Sciences
• One (1) year of experience in mental health services
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
