Salary: $27.47 - $30.22/hr

Position Summary: Family Stabilization (FS) Case Planners are responsible for providing culturally competent, short-term Crisis Intervention treatment to families seeking Family Stabilization services in the boroughs of the Bronx, Manhattan, Brooklyn and Staten Island.

Essential Duties:



  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Maximum Caseload size of approximately 10 FS cases.
  • Attend all FS trainings as required as well as all mandated ACS trainings.
  • Fulfill all OCFS, and ACS accountability requirements including timely completion of FASPs, Progress Notes and Plan Amendments.
  • Perform system updates on PROMIS, CONNX and eVOLV.
  • Contact families within 24 hours of the referral in order to schedule initial intake appointment.
  • Schedule and meet with families for initial intake appointment within five (5) calendar days of referral.
  • Maintain an average of 3 case work contacts per month per family as specified by the model.
  • Meet with all families and all children every month.
  • Be available by phone 24/7 or on weekends as needed to respond to client crises.
  • Weekend coverage is required for sessions as needed, to accommodate client availability.
  • Conduct home assessment as well as assessment of financial resources for families.
  • Assist families with concrete needs and services, educational advocacy and referrals to support services to alleviate barriers to their participation in treatment.
  • Assist with concrete case management tasks, e.g. rent arrears, utility shut off, furniture assistance.
  • Collaborate with Educational and Employment specialist to monitor school attendance, obtain school records and intervene with DOE when appropriate to develop an appropriate education/vocational plan for youth.
  • Research and identify appropriate community-based services to support families, e.g. mental health programs, DV programs, pro-social activities for adolescents etc.
  • Make referrals to community-based organizations/agencies as needed.
  • Engage in outreach activities in the community in order to attain required 20% utilization from self-referrals as required by contract.
  • May be required to accept cases in other boroughs where we have contract for Family Stabilization Programs (Bronx, Brooklyn, Manhattan, and Staten Island).
  • Ability to work flexible hour, including working remotely in the field, community or various CA offices to meet the needs of families and improve efficiency and management of time and resources.
  • Perform other duties as assigned.



      Minimum Qualifications:



      • Bachelor's degree in social work or related field required.
      • Experience working with adolescent and/or families preferably in a child welfare, juvenile justice or mental health settings.



      Key Competencies:



      • Bilingual in English and Spanish preferred.
      • Good organizational skills with ability to meet deadlines.
      • Ability and willingness to work with families and teens in the home, school and other appropriate settings in the community.
      • Ability to work flexible hours including evenings and some weekends as needed.
      • Ability to work independently, as well as contribute to a team.
      • Strong clinical skills and ability to work under pressure with high risk, crisis-oriented cases.
      • Very good interpersonal/communication skills, including ability to write well and meet copious amount of documentation in a timely manner.
      • Computer literate.
