Job Description
General Purpose of Job: This role primarily collaborates with the Medical Director, Director and Managers of Hospice and Palliative Care to ensure quality care is provided for patients. This Supervisor role will also collaborate with the UMH-Sparrow Performance Improvement Specialist. Key functions of the role include data collection, entry, submission and reporting as required. Additionally, external data sources such as Hospice Community Assessment of Healthcare Providers (HCAHPS) are monitored, data is evaluated, and education is provided as applicable to promote quality patient care. Supports leadership and team members as needed in meeting department and organizational goals. This includes but is not limited to providing education, validating competencies, doing audits and needs assessment, and building relationships with community partners to enhance and grow services. Provides hospice leadership on-call coverage as coordinated with other department leaders. - Educates Nurses and Certified Nurse Aides in collaboration with their leadership team members..
- In coordination with the leadership team, provides supervisory visits, gives feedback on performance and offers coaching and teaching.
- Works closely with other department leaders by responding to concerns, clinical issues and learning/growth opportunities.
- In collaboration with other leaders, helps select and orient team members for positions with Home Hospice and Hospice House. Trains, supervises, counsels and evaluates all team members on orientation as requested by Manager/ Director. Provides team members with support, encouragement and motivation to perform job duties in optimal manner. Manages and ensures regulatory compliance with all orientation documentation and on-going education requirements.
- Assists in development and helps maintain department policies.
- Participates in department Performance Improvement including Hospice Quality Outcomes from Ability and CMS Hospice Compare, Infection Control initiatives, Corporate Compliance, Patient Satisfaction, Hospice Caregiver Engagement initiatives and monthly reporting of key operational information. Identifies opportunities for improvement. Plans and implements strategies to improve department operations and outcomes in collaboration with Sparrow Quality Department, Managers and Director.
- Provides education to department staff, physicians, other hospital departments and others related to department operations, responsibilities, and services.
- Assists in the development of department goals and objectives. Helps implement plans to achieve goals. Participates in efforts to support and advance the mission, vision, strategies and goals of the Health System.
- Monitors customer satisfaction, performance improvement and corporate compliance indicators, and key indicators of effectiveness and efficiency to ensure optimal performance.
- Assists in the development and implementation of special projects.
- Maintains proficiency in computerized documentation practices and can mentor others as they learn software documentation.
Job Requirements
| General Requirements |
* Current Michigan Registered Nursing License. * BLS through American Heart Association. * Active, valid State of Michigan Driver's License. * Proof of active car insurance. |
| Work Experience |
* Minimum of three years experience working in a registered nurse capacity. * Preferred - One year of Quality / Performance Improvement experience. * Preferred - One year of educator/teaching experience. * Preferred - Experience in completing Hospice data sets and Advance Care planning. * Preferred - Experience with Epic applications for Hospice services (Hyperspace, Comfort, Remote Client). * Preferred - Experience with Ability, Press Ganey, RL Solutions and / or CMS Hospice Compare software/ websites. * Preferred - Leadership experience. |
