Hiring Range:

$46.09 - $82.97

The ED pharmacist shall optimize medication use through participation in direct patient care rounds, medication order review, medication therapy monitoring, participation in procedures that utilize high-risk medications, resuscitation, medication procurement and preparation, provision of medication information and documentation of associated interventions.

I. Major Responsibilities:

1. Medication order review

  • a)Review all medication orders generated within the ED, either real time or as retrospective review.
  • b)Provides feedback to prescribers for any orders identified as possible interventions
  • c) compiles information from retrospective review of orders to improve quality of care and shares with the ED staff
  • d) Develop a triage process for order review to prioritize high risk medications, high risk populations and emergent situations

2. Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols.

  • a)Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug, dose and route of administration.
  • b)Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
  • c)Provides clinical consultation and clarification to practitioners. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
  • d)Provides accurate, adequate and timely drug information to the professional staff.
  • e)Documents all clinical activities and interventions accurately and completely.
  • f) Evaluate current processes for high risk medications and assist in the development of processes to improve current practice

3.Resusitation.

  • a)Attend all resuscitations, procedural sedations, strokes, STEMIs,etc
  • b)Prepare medications for immediate administration
  • c)Ensure appropriate medication and dose selection
  • d)Ensure appropriate administration of medications
  • e)Obtain all medications not readily available
  • f) Recommend alternate routes of administration when necessary

4.Education and discharge planning

  • a)Work closely with the ED Nurse Educator to develop programs to address the needs of the ED staff based on the proactive and continuous monitoring of medication practices as well as new additions to formulary
  • b) Facilitate discharge planning and access to medications in an ambulatory pharmacy for patients.
  • c) provide counselling to ED patients prior to discharge regarding any new medications

5.Facilitate procurement of medications on weekends when main pharmacy is closed

  • a) serve as contact person for Nursing when a new medication is ordered and is needed on the nursing unit.
  • b) Ensure medication is obtained and provided to nursing unit

II. Position Qualifications:
License/Certification/Education:
Required:
1.Graduate of an ACPE-accredited School of Pharmacy

2. Currently licensed to practice pharmacy in the state of Massachusetts
Preferred:
1. Pharm D preferred

Experience/Skills:
Required:
1. Health system experience required
Preferred:
1.Emergency Medicine experience preferred

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
