Rutgers, The State University of New Jersey, is seeking a Resource Coordinator for the department of Medicine at the New Jersey Medical School.
The Resource Coordinator plays a critical role in supporting the success of the patient navigators working within the Routine
HIV and
STI Linkage Program in the Emergency Room and Greater Newark Community via community engagement activities. This position assists with quality assurance and quality improvement activities, chart reviews, training, and program efficiency. The Resource Coordinator also ensures adherence to protocols, supports data tracking and documentation, and facilitates coordination among patient navigators, medical providers, and community partners. The role is designed to deliver effective linkage to care for clients across the
HIV status spectrum-newly diagnosed, at risk, or living with
HIV.
Among the key duties of this position are the following:
- Provides direct day-to-day support to ensure clients are effectively linked to HIV/STI prevention, care, or treatment services.
- Supports onboarding and ongoing training of staff within best practices related to linkage to care, harm reduction, data entry, and client engagement.
- Orients clients and provides information about the available services of the ID Practice.
- Monitors and tracks client progress and compliance with medical/lab requirements and treatment plan.
- Answer client telephone calls. Correctly processes information for appointment scheduling, answer questions, and assist with problem solving. Screens emergent/urgent phone calls, inquiries related to the availability of appointments, scheduled appointment dates and times. If necessary, transfers calls to an appropriate person or department to assist the client.
- Ensures referrals are scheduled in a timely manner documenting results to clinic SN PNs, MCM and physician.

Qualifications
| Minimum Education and Experience |
- Bachelor's Degree in a Health, Public Health, Social Work, or other relevant field required, plus three (3) years of relevant experience, particularly in HIV/STI services, case management, or patient navigation.
- Equivalent education, experience, and/or training may be substituted for the degree requirements.

- Five years of experience preferred.
NJDHSS HIV counselor certification preferred.- Knowledge of Newark community preferred.
- Bilingual skills (English/Spanish) preferred.

- Physical Demands: Computer literacy, typing, working at multiple workstations in a shared environment, ability to speak clearly, write proficiently. Extensive reading and extended sitting, ability to move from unit to unit. Position requires standing and walking over 2/3 of the time. Talking, hearing & sight are essential; requires clear close & distance vision, peripheral & color vision, depth perception and ability to adjust focus. Position requires lifting to 10 lbs.; on occasion more based upon client demands. Assist with transportation services in the clinic.
- Work Environment: Occasional (less than 1/3 of time) exposure to bloodborne pathogens that requires use of personal protective equipment.
