Job Description Summary: The Ambulatory Care Pharmacist will play a key clinical leadership role in Spira Care, a rapidly growing, integrated primary care organization aimed at improving health outcomes, lowering medical costs, and improving the overall patient experience.
This role provides patient care through the provision of direct operational and/or clinical patient services. The pharmacist will collaborate within the department to enhance departmental operations and clinical programs. Managing programs and people, this position directly impacts patient care and quality outcomes of populations, playing a critical role in the development of clinical pathways, best practices, and workflows directed at Quintuple Aim objectives at the individual and
population level and in collaboration with an interprofessional team.
Job Description:
* Develop and maintain collaborative relationships with physicians, patients, and other healthcare team members within Spira Care and Blue Cross and Blue Shield of Kansas City. * Provide comprehensive medication management and education for Spira patients, to include lab review, clinical chart review, education on glucometers, hypoglycemia, diet, exercise and other medications, and clinical documentation; ensure patients have access to affordable medications. * Provide standardized screenings for depression, diabetes foot exams or other screenings to promote health and wellness. * Participate in evidence-based activities related to improving outcomes for targeted disease management initiatives leading to improved health. * Identify opportunities for cost savings at the individual patient level and the population level that are in keeping with best practices. * Provide education and training support to healthcare teams in the broad area of pharmacotherapy. * Demonstrate leadership and pharmaceutical excellence through patient care activities as a member of a multidisciplinary healthcare team. * Provide direct supervision of clinical support team based on chronic disease conditions. * Effectively communicate with the healthcare team regarding patient care, assessment, and education issues identified through the comprehensive medication management process. * Participate in quality improvement initiatives, population health programs, assigned committees, and other development opportunities established by the organization. * Effectively use clinical and health plan data to drive towards desired outcomes. * Ensure confidentiality of all patient and organizational information in accordance with HIPAA legislation, protected health information (PHI), and Blue KC policy. * Meet organization-wide quality metrics year over year. * Travel in Greater Kansas City Metropolitan area as required. * Other duties as assigned Minimum Qualifications * Degree in Pharmacy from an accredited College of Pharmacy * Completion of an accredited PGY1 pharmacy residency program or three years of previous clinical pharmacy experience in the ambulatory care setting * Board Certification in Ambulatory Care or within one year of hire * Current and unrestricted pharmacy licensure in Missouri and Kansas * Strong organizational skills * Strong problem solving skills * Computer skills * Strong written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts Preferred Qualifications * PGY2 pharmacy residency program in ambulatory care * American College of Clinical Pharmacy (ACCP) Training Spira Care is an equal opportunity employer.
