JOB DESCRIPTION:

SUMMARY OF POSITION

Responsible for engagement of hospitalized patients who are being discharged for follow up care at Centerstone. Hospital Liaisons will work with local hospital staff to build partnerships with Centerstone to ensure that discharging patients become fully linked with outpatient services after discharge. The Hospital Liaisons will also serve as the point person for the local hospitals and be flexible and available to the hospitals when needed by cell phone or in person.

Hospital Liaisons will assist the Centerstone Health Link team in making Health Link Care Coordination calls as needed. They will also be available to Team Leader and all Centerstone staff via Skype online during work hours when not in the field.

Supports the Centerstone Mission, Vision and Values in delivering care that changes people's lives. Communicates clearly and appropriately; provides excellent customer service. Adheres to applicable policies and procedures. Regular attendance and punctuality is required.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Coordinate with area hospitals to ensure patients discharging to Centerstone are linked to outpatient follow up care.
  • Ensure that all documentation is completed in the Electronic Health Record.
  • Ensure that appropriate follow up with clients is occurring.
  • Coordinate with other Centerstone staff to ensure clients have a smooth transition from inpatient to outpatient services.
  • Document and maintain data tracking in an excel file
  • Compliance with annual KPI evaluations
  • Other duties as assigned

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Excellent skills with Excel.
  • Exceptional organization skills
  • Ability to coordinate multiple tasks

QUALIFICATIONS

Education Level

Bachelor Degree required in psychology, social work or related field preferred.

Years of Experience

Two years of experience working with individuals with serious mental illness preferred.
