Requirements
- License State LPN license
- Experience in IDD and knowledge of licensure regulations preferred,
- Familiarity of medicare system and durable medical equipment purchasing a plus.
- Criminal Clearance, Child Abuse Clearance as needed, valid Driver's License.
- Knowledge of DW 6400 regulations, familiar with medicare system and medical insurance.

We are seeking Licensed Practical Nurse to join our team in our Hummelstown, PA location.
Position Details:
- Administers nursing care, including medications, in alignment with treatment plans and in accordance with physician's orders, under supervision.
- Communicates and collaborates with physicians or specialists regarding patient care and to meet preventative health standards.
- Educates patients about health maintenance and disease prevention.
- Provides advice and emotional support to patients and their family members.
- Observes, interprets, records, and reports any signs, symptoms, and/or processes that change and/or reflect actual or potential health problems.
- Provides health promotion, training, education, and maintenance through teaching, guidance, and counseling the residents, families, Direct Support Professionals, and other members of staff.
- Identifies actual and potential safety hazards within the assigned care area and takes appropriate action to eliminate them.
- Assists in coordination, collaboration and follow through regarding specialist clinics (podiatry, psychology, vision, and dental) as well as transcribing orders upon client's return from specialists.
- Maintains accurate patient medical records.
- Performs on-call responsibilities as needed.
- Performs other duties as assigned.
