About the Role

The Direct Care Professional plays a vital role in providing compassionate and personalized support to individuals in need of assistance with daily living activities. This position focuses on enhancing the quality of life for clients by promoting independence, dignity, and respect in a safe and supportive environment. The role requires close collaboration with healthcare providers, family members, and other team members to ensure comprehensive care plans are effectively implemented. The Direct Care Professional will be responsible for observing and reporting changes in clients' conditions to facilitate timely interventions. Ultimately, this position aims to empower clients to achieve their personal goals and maintain their well-being within the community setting at 155 Clearview Road-6068W.


Job Responsibilities

  • Assist clients with activities of daily living such as bathing, dressing, grooming, and meal preparation.
  • Provide emotional support and companionship to promote social engagement and mental well-being.
  • Monitor clients' health status and report any changes or concerns to the supervisory healthcare team.
  • Implement individualized care plans and support clients in achieving their personal goals.
  • Maintain accurate documentation of care provided and communicate effectively with team members and family.


Minimum Qualifications

  • High school diploma or equivalent.
  • Ability to pass a background check and drug screening.
  • Basic knowledge of personal care and safety procedures.
  • Strong communication and interpersonal skills.
  • Physical ability to assist clients with mobility and daily tasks.


Preferred Qualifications

  • Previous experience in a direct care or caregiving role.
  • Certification in CPR and First Aid.
  • Training or coursework related to healthcare, social work, or human services.
  • Familiarity with electronic health records (EHR) systems.
  • Ability to speak multiple languages relevant to the client population.

The Direct Care Professional utilizes strong interpersonal and communication skills daily to build trusting relationships with clients and their families. Attention to detail and observational skills are essential for monitoring client health and reporting changes accurately. Physical stamina and coordination are necessary to assist clients safely with mobility and personal care tasks. Problem-solving and adaptability help the professional respond effectively to the dynamic needs of clients. Additionally, organizational skills support maintaining thorough documentation and coordinating care with the broader healthcare team.
