~ 40 Hours/week, Full-Time, Day shift ~

Maine MHRT/C certification is required

We are looking to add a Case Manager to our Adult Services team. Our experienced case managers meet with patients and families to assess their needs. They also connect patients and families to resources and services that can support patient independence and quality of life. Our services promote physical and mental wellness as well as individual independence and success in their community.

* Connecting patients and families with community resources

* Helping patients and families navigate the social services and health care system

* Coordinating services and communication between multiple providers

    * Developing and monitoring individualized treatment plans

    Summary

    The Behavioral Health Case Manager - Adult Services role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients.

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Bachelor's Degree from an accredited institution with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, or closely related field preferred.
    2. License/Certifications: MHRT/C required. Valid Driver's License and appropriate level of automobile liability insurance required.
    3. Experience: One year of experience working with a broad spectrum of clients with mental health diagnoses required. One year of experience in the same areas of specialization required. Experience with clinical methods and techniques used in supportive counseling as related to mental health and/or substance abuse issues required.
