*Job Title: Member and Provider Outreach Coordinator*
*Placement Type:* Contract (17 weeks) *Expected Start Date:* November 3, 2025 *Pay Rate:* *$19.00/hour * *Work Environment:* Fully Remote (Excludes CT, PA, OH, SD, NM, HI, WV, TN, DC, and CA) *Job Summary:* The Member and Provider Outreach Coordinator plays a vital role in supporting and HEDIS performance measures. This position focuses on member engagement, provider communication, and medical record retrieval to close gaps in care and improve healthcare outcomes. * * *Key Responsibilities:* * Conduct outreach to members to address barriers to care, educate on missing gaps, and assist with scheduling appointments. * Document member interactions in systems and outreach tracking spreadsheets. * Follow up with members as needed, including appointment reminders. * Contact providers to obtain medical records for services rendered to members. * Send communications via fax and email to provider offices and perform timely follow-ups until records are received. * Accurately document medical record retrieval activities in applicable systems. * Respond to inquiries from members, providers, hospitals, and medical record vendors via telephone. * Coordinate with internal departments to ensure timely and appropriate responses. * Escalate complex medical record retrieval issues when necessary. * Assist in scheduling and coordinating team activities as needed. * Perform additional administrative duties as assigned. *Required Skills and Qualifications:* * Excellent customer service and phone communication skills. * Proficiency in computer-based work; ability to perform all tasks digitally. * Strong attention to detail and accuracy. * Experience in customer service, clerical support, member/provider outreach, or provider relations. * Familiarity with medical records, Release of Information processes, and medical records departments. * Effective verbal and written communication abilities. *Employee Value Proposition:* * Opportunity to apply internally for full-time roles. * Supportive leadership and collaborative team environment. * Exposure to diverse projects including member outreach, provider engagement, and medical retrieval audits. * Letter of recommendation available based on performance and work ethic. *Business Impact:* * Supports preventative care by ensuring members attend scheduled appointments for outstanding healthcare needs. * Drives data collection and medical record retrieval to enhance HEDIS star ratings. *Pay and Benefits* The pay range for this position is $19.00 - $19.00/hr.
