Family Services Coordinator

Washington County, located in West Bend, Wisconsin, has an excellent opportunity for a full-time Family Services Coordinator. Under the general direction of the Youth Treatment Services Supervisor, this position is responsible for all intake, assessment, treatment planning, and progress note documentation for each family unit. Wraparound services include finding resources to assist in stabilizing housing, securing employment, furthering education, connecting with follow-up treatment and other areas of need in the person's life. Coordinator works collaboratively with Children and Families and Behavioral Health staff to provide the most clinically appropriate services to match client need and preference for ongoing recovery.
Qualified candidates must possess:
*A Master's Degree in Social Work, Psychology, Criminal Justice or related field.
*Previous experience working with child welfare, case management with individuals with substance use and/or mental health issues, and family systems preferred.
*Knowledge of systems and programs, applicable state laws and regulations
*A valid Wisconsin driver's license, available automobile, and automobile insurance are required.

Salary range: $69,779.00-$79,547.99

Position Summary:
This position will provide services for families that are at risk of child removal with child welfare due to caregiver substance use. Under the general direction of the Youth Treatment Services Supervisor, this position is responsible for all intake, assessment, treatment planning, and progress note documentation for each family unit. Wraparound services include finding resources to assist in stabilizing housing, securing employment, furthering education, connecting with follow-up treatment and other areas of need in the person's life. Coordinator works collaboratively with Children and Families and Behavioral Health staff to provide the most clinically appropriate services to match client need and preference for ongoing recovery.

Duties/Responsibilities:

The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

*Conduct an assessment to evaluate both the strengths and needs of the family.

*Analyze situations, capabilities and problems to determine what services are required to meet the needs of the family.

*Coach the family as they learn to navigate systems and advocate for themselves to remove barriers.

*Address the basic needs of the family, in addition to any therapeutic intervention, to increase effectiveness on issues such as housing, energy assistance and job security.

*Formulate, implement, evaluate and revise goal oriented and strength-based treatment plan.

*Develop a crisis/safety plan for the family

*Maintain contact with family.

*Coordinate with other service providers and community resources to prevent overwhelm and encourage family success.

*Research resources and utilize them to assist the client in meeting their goals.

*Safely transport clients as needed.

*Performs other duties as assigned, including responding to an emergency event.

Work Direction Received:
Works under the general direction of the Youth Treatment Services Supervisor.

Supervision Exercised:
None

Decision Making:
Decisions concerning service authorization, treatment plans, and daily activities are made independently.
Interaction:
There is significant interaction with clients, other County Departments, State officials, area health care providers, and the general public.
Essential Knowledge and Abilities:
Must possess the skills to build trusting relationships with enrolled families. Previous experience working with child welfare, case management with individuals with substance use and/or mental health issues, and family systems preferred. Knowledge of systems and programs, applicable state laws, and regulations. Commitment of care of the clients and their family, communicate effectively orally and in writing, and interact effectively with other health care providers and the general public. Able to fulfill physical demands of job, sensory demands (seeing and hearing), and cognitive demands (concentration, conceptualization, memorization).

Training and Experience:
Master's Degree in Social Work, Psychology, Criminal Justice or other related field. Must possess a valid Wisconsin driver's license.
