Description

We are looking for an experienced Wound Care Nurse (LPN/RN) to work in Avon, Ohio at the Saint Therese/St. Mary of the Woods campus. This campus is a senior living community that offers a variety of housing options and levels of care, including independent living, assisted living, skilled nursing, short term rehabilitation and respite care.

St. Mary of the Woods was built with a deep Catholic foundation, and it has an impeccable reputation for quality care in the marketplace. Built in 2005, it has 179 total units with 81 independent living apartments, 48 assisted living apartments and 50 rooms in skilled nursing and transitional care. It is a five-star facility licensed by Medicare and Medicaid, recognized as a 2024 Best Nursing Home by U.S. News and World Report and is open to people of any faith tradition.

Responsibilities:

The Wound Nurse is responsible for:



  1. Assessing and Diagnosing Wounds: evaluate the severity and type of wounds of the resident, diagnose appropriate treatments, and develop personalized care plans tailored to each residents' needs.
  2. Wound Cleaning and Dressing: responsible for cleaning wounds and applying suitable dressings, ensure that wounds are protected from infection and other complications, and promote a safe healing environment.
  3. Monitoring and Documentation: monitor the progress of wound healing and document changes in the resident's condition. This includes noting any signs of infection or complications that may arise during treatment.
  4. Patient Education: Educating patients and their caregivers about proper wound care techniques, infection prevention, and the importance of following treatment plans is a crucial part of the role.
  5. Collaboration with the healthcare team



Qualifications



  • Successful completion of an approved nursing program for Registered Nurses or Licensed Practical Nurse
  • Currently licensed in Ohio and in good standing.
  • Relevant certifications in wound care
  • Active CPR certification


Experience:



  • At least three years of successful nursing experience in a senior care/skilled care environment; with prior experience in wound care
  • Knowledge of legal and regulatory healthcare requirements of a skilled nursing environment.

    • Must be competent and willing to educate others.
    • Willingness to utilize, participate in, adapt and realign work using the PDSA cycle (Plan, Do, Study, Act)
    • Proficient computer skills with Microsoft Office applications and other required software programs
    • Must be able to function independently, demonstrate flexibility.
    • Ability to manage multiple tasks/projects and deadlines simultaneously.
    • Understand, interpret, and utilize data to improve resident care.
    • Ability to interact with all stakeholders from varying social, cultural and educational backgrounds.
    • Excellent interpersonal communication, and organizational skills both written and verbal.
    • Experience with various office equipment
    • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
    • Desire for continuous growth and development
