The HVC Clinical Services Facilitator (CSF) functions primarily as a department liaison to the referring physician office/clinic to facilitate patient scheduling. As appropriate, consults with section supervisors/coordinators, radiologists, technologists, anesthesiologists, nurse professionals, business services staff,
and schedulers to coordinate appointments for procedures and related tasks to achieve timely and efficient patient care.
The CSF is responsible for accurately aligning activities and making sound decisions related to sequencing of complex procedures, clinic appointments and/or OR procedures in coordination with physician schedules or multiple patient appointments. To that end, they are expected to be competent with various computer based healthcare applications, familiar with requirements of all imaging modalities, including the average length of each scan /procedure as well as patient preps, and have an appropriate understanding of basic anatomy.
The personal attributes necessary for this position require exceptional interpersonal /communication skills, attention to detail, documentation and organizational skills, self-motivation, and good independent judgment. To meet the staffing needs of the department, as necessary. The Clinical Services Facilitator serves as a point of contact for the practice sites for Heart and Vascular Ambulatory Programs in support of the managers for the clinical practice sites and the HVC Clinical Program Director.
The Clinical Services Facilitator collaborates with service line and department leadership to design, develop, and implement clinically and fiscally responsive program philosophies, goals, and objectives. Under the direction of the department leadership, Facilitates the development and implementation of core processes and systems of care which achieves strategically relevant volume and quality outcomes in the area of cardiovascular medicine.

Responsibilities
- 1. Coordinates and supports all tasks related to Heart and Vascular support and patient scheduling.
- 1.1 Responds to clinical questions and concerns around the appointment/procedure to ensure that all
inpatient and outpatient visits are scheduled appropriately.
- 2. Facilitates smooth and timely workflow in conjunction with the section coordinator /supervisor to ensure
resources are filled to capacity.
- 2.1 Ensures all inpatient and outpatient exam requests are scheduled in a timely manner as
appropriate for the section.
- 3. Ensures all procedures are scheduled with optimum customer service.
- 3.1 Greets patients using excellent customer service skills and discretion following YNHH and department
guidelines
- 4. Assists manager/coordinator with regulatory readiness and compliance
- 4.1 Provides information to referring MDs regarding proper exams and CPT codes
- 5. Works with manager/coordinator to ensure all Quality Assurance measures are met.
- 5.1 Monitors the scheduled exams report for exams that have not been completed or cancelled from the
previous day.
Qualifications
EDUCATION (number of years and type required to perform the position duties): Associate degree in Business or Healthcare Sciences / Administration with medical terminology, hospital/clinic or equivalent healthcare experience preferred. Ongoing continuing education related to correct coding and understanding of regulatory compliance is encouraged. EXPERIENCE Minimum of three (3) years as a Clinical Services Assistant or equivalent position with direct experience coordinating and facilitating patient appointment scheduling in a healthcare environment. Individual must have computer experience in such applications as Word, Excel, and PowerPoint etc. Individual must have previous experience or education related to Third Party Payor requirements, CPT coding, and medical necessity as it pertains to the Imaging session. LICENSURE N/A SPECIAL SKILLS Highly developed administrative, communication skills are essential. Excellent interpersonal skills with the ability to effectively communicate with referring physicians, nursing/scheduling professionals, or directly with patients to assess their needs in order to maximize coordination of care. Individual must have critical thinking skills and analytical abilities as it relates to assessment of clinical needs, exam urgency, and appointment sequencing using computer skills to assist with decision support, as well as the problem solving skills to make decisions independently within position guidelines and expectations. Maintain Database of referrals & structural procedures to allow for structured and focused investments in programmatic growth. Adhering to all guidelines and timeframes in accordance with CMS and various registries. Collaborate with physician leaders to coordinate fellowship candidates and assist with on-boarding; credentialing, re-credentialing and re-appointments as needed with University staff. Assume responsibility for managing Structural calendar, invites, zooms for the various team meetings; Multidisciplinary conferences, specialty monthly meetings, Journal Club, Structural Attending Heart Conference schedule, M&Ms, and QAPI Assist and collaborate with SH nursing coordinators to ensure cohesive clinic schedules that align with physicians assignments throughout the monthly Qgneda schedule for NH, Waterford clinics. PHYSICAL DEMAND Ability to lift 25 pounds. supplies etc.
