Position Overview

The MDS Nurse Assessor is responsible for collecting and submitting assessment data critical to the health and well-being of our residents. You will support the MDS/PPS Coordinator in completing PPS and/or OBRA assessments for both subacute and long-term care residents, as well as perform additional tasks as assigned.

RN - MDS Nurse Assessor - Essential Job Functions:

• Accurately and timely complete and submit Minimum Data Set (MDS) assessments

• Ensure compliance with Federal, State, and facility policies

• Actively participate in utilization management processes, including PDPM and state case mix systems

• Conduct resident interviews

• Compile, monitor, and audit clinical documentation

• Communicate clinical issues effectively to appropriate clinicians

• Support and assist the MDS/PPS Coordinator as needed

• Demonstrate thorough knowledge of policies and procedures

• Utilize electronic timekeeping systems as directed

• Follow facility dress code and identification policies

• Abide by all policies related to electronic devices, computers, and tablets

• Maintain compliance with company code of conduct, HIPAA, and regulatory standards

• Perform other duties as assigned by supervisor or Clinical Reimbursement Department

RN - MDS Nurse Assessor - Qualifications:

• Licensed Professional Nurse (RN licensure preferred) with current state license

• Prior experience with MDS 3.0 preferred

• Long-term care experience preferred

• Excellent verbal and written communication skills

• Self-motivated, highly organized, and dedicated

• Willingness to train the right candidate
