You will serve as a licensed, substance abuse expert on an interdisciplinary Assertive Community Treatment (ACT) clinical team working with individuals diagnosed with a Serious Mental Illness or General Mental Health diagnosis under Court Ordered Treatment. Responsibilities:

Seeking an ACT Counselor to join our interdisciplinary ACT clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our 23rd Ave Healthcare Clinic. If you're a licensed Clinician, but would rather be out in the community than behind a desk, this is the role for you!

Full-Time Employed: 40 hours/week

Location: 23rd Ave and Dunlap in Phoenix, AZ

Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Must have very flexible schedule as this team provides treatment 7 days/week, including weekends and holidays

Generous PTO/Sick Time (4+ weeks in year 1)

$3/hour ACT Differential

The ACT Counselor Develops program activities, facilitates teamwork for outpatient co-occurring disorders services, and conducts individual sessions and classroom-style group treatment services. Works as a team member of an ACT clinical team to assess member needs and goals, participates in the development of individual service plans, acts as a member advocate and liaisons with families and community agencies. Utilizes Motivational Interviewing techniques to effectively engage members into treatment services. Serves as the ACT team's expert for substance use issues by providing other team staff with education and consultation in respect to substance use issues. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Works under the direct supervision of the ACT Clinical Coordinator.

Works as a team member of an ACT clinical team to assess member needs and goals, participates in the development of individual service plans, acts as a member advocate and liaisons with families and community agencies.

Utilizes Motivational Interviewing techniques to effectively engage members into treatment services.

Provides time-unlimited (O7), community-based services (S1) to all ACT team clients served on the ACT team, including but not limited to: Assessment of client needs and level of functioning, developing individual treatment plans, providing direct care, coordinating services across providers, advocating for client, medication observation and education, independent living skills training, and identifying and procuring meaningful daily activities

Actively recruits new clients who could benefit from ACT services

Routinely assesses client need for ACT services, using explicit criteria to determine need to transfer to less intensive services. Gradually and individually transitions eligible clients and ensure continuity of care.

Monitors client status during transition and reintroduces services as needed to ensure client stability in the community.

Facilitates substance use treatment, both individually and in groups, per clients' individual service plan. Provides other therapeutic counseling services to ACT clients, based on clients' individual needs and treatment recommendations.

Participates in the service plan development process for each client as it relates to their recovery and the interventions and services they will need.


Qualifications

Education and/or experience may run concurrently

MA in Behavioral health or related field

1 year experience in behavioral health required

Must meet the requirements of a Behavioral Health Professional in the State of Arizona as LMSW, LCSW, LAC, LPC

Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred

Must have a valid Level 1 Arizona Fingerprint Clearance card or the ability to apply for one within 7 working days of assuming role

Must be 21 years of age or older, with a valid Arizona driver's license, and a minimum of 3 years driving experience and meet requirements of Terros Health's driving policy

Strong understanding of substance use, addiction and recovery clinical skills

Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members

Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions

Must be able to to pass a TB test
