At Henry Ford Home Infusion, our professional staff are dedicated to providing quality individualized home infusion and nutritional management services. Services are ordered by a physician and offer a safe and effective alternative to hospital treatment and allow patients to resume their normal work activities and lifestyle in a more-timely manner. Ideal candidates will have experience with Home Infusion patient referrals, specifically for enteral (tube-feeding) and parenteral (IV-TPN) nutrition. A strong ability to collaborate effectively with hospital staff, case managers, and the RPH team is essential to ensure optimal patient outcomes. Successful individuals in this role will demonstrate attention to detail, a commitment to patient-centered care, and the ability to navigate complex referral processes with ease. GENERAL SUMMARY: Under general supervision of the Home Infusion pharmacy manager and/or nursing supervisor, the Registered Dietitian is responsible for assessing nutritional status and risk identification of enteral and parenteral nutrition patients and coordinates all aspects of patient home nutritional therapy. EDUCATION REQUIRED: Bachelor's degree in Food/Nutrition or Dietetics and an approved dietetic internship required. EXPERIENCE REQUIRED: Minimum of three (3) years of clinical experience is required. One (1) year of home infusion or home health care experience preferred. Knowledge of enteral formula conversions required. Knowledge of home infusion policies and procedures preferred. Knowledge of Medicare/Medicaid enteral nutrition requirements preferred. Knowledge of Insurance payer requirements and coverage preferred. Strong interpersonal skills are required to communicate effectively with medical personnel and staff; to obtain information from and explain dietetic restrictions to patients and families; and to design and conduct educational programs for Home Infusion team. Must have excellent written and verbal customer service skills to interact with all levels and types of patients, customers and team members. Must be proficient with multiple and diverse computer applications and use of a computer. Must have reliable transportation and a telephone. CERTIFICATIONS/LICENSURES REQUIRED: Registration with the Commission on Dietetic Registration required. Membership in the American Dietetic Association preferred. Must possess a current State of Michigan driver's license and a satisfactory driving record.
