Job Summary:
|
The Patient Transition Specialist is a professional whose duties and responsibilities are to support patients and their families as they move their care from our pediatric clinic to adult care at another institution. This would include, but is not limited to: Key Responsibilities:
- Create comprehensive resource lists and internal tracking documents for adult care providers and other nonclinical needs, ensuring accurate and accessible information for patient needs
- Collaborate with TRUE team members and programs across the state and region to identify and address unique, non-clinical patient needs to include but not limited to providing resources (internal and external) and guidance to meet desired goals, assistance in finding an adult provider when ready and other non-clinical needs that arise and are within clinic's scope
- Meet with patients and families during and after clinic appointments to independently assess needs, provide guidance, and coordinate necessary support services, demonstrating sensitivity and empathy in patient interactions.
- Verify and maintain accurate information about internal and external/community programs, ensuring they align with patients' specific medical and personal requirements while adhering to healthcare regulatory standards.
- Serve as a key liaison between pediatric clinic staff, adult care providers, patients, families, and community organizations, fostering clear and timely communication to enhance patient care continuity.
- Advocate for patients' preferences and concerns throughout their time in TRUE, and as they begin the transition to adult care process, maintain high ethical standards and compliance with healthcare regulations in all aspects of patient transition coordination, ensuring confidentiality and patient safety.
- Independently undertake supplementary coordination tasks as necessary to support smooth patient transitions, including administrative duties, logistical arrangements, and ongoing evaluation of transition processes.

Qualifications:
|
Minimum Qualifications:
- Bachelor's degree in any field
- 3-4 years of experience with care coordination, case management, or community outreach*
- 2 years of patient-facing experience in a clinical setting*
*Experience may be obtained simultaneously Substitution: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year-for-year basis. Applicants must meet minimum qualifications at the time of hire. Condition of Employment:
- Must successfully pass a drug test through Children's Hospital Colorado
- Must be willing and able to pass a national criminal background check

- 2+ years of experience working in a clinical setting
- Experience working with adolescents and/or young adults
- Experience with Epic EMR system
- Experience working with transgender and gender diverse youth
Knowledge, Skills, and Abilities:
- Critical thinking: Assessing and evaluating individual patient needs to determine the most suitable solutions based on various factors such as location, insurance, co-morbidity, neuro-divergence, etc.
- Utilize analytical, evaluative, and interpretive skills to understand and address the needs of individual patients and evaluate adult programs effectively.
- Ability to apply expertise gained from experience or education to effectively communicate with adult programs and ensure comprehensive information gathering before, during, or after transitions.
- Ability to establish and maintain effective working relationships with employees at all levels throughout and outside the institution.
- Outstanding customer service skills.
- Ability to communicate effectively, both in writing and orally.
- Outgoing, organized, flexible, detail-oriented, and able to work independently to serve our families

Anticipated Pay Range:
|
The hiring range for this position has been established as $66,537 to $84,635. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level. Your total compensation goes beyond the number on your paycheck.
