Position Summary

The Welcome Baby (WB) Program is a free, voluntary home visitation program which includes visits with the family prenatally, in the hospital, and during the postpartum period. The purpose of the program is to provide primary health prevention, parent education, and to invite and welcome families into an array of services and support elements developed in their community. As a key member of the team, the hospital liaison will be based in the hospital setting, working to engage all families after delivery, and enroll new families into the program. They will work closely with the WB nurse and Parent Coaches to ensure continuation of services after maternal/infant hospital discharge. Responsibilities include conducting an assessment with each eligible client, observing parent infant interaction; assessing involvement of partner, level of social support, infant feeding, maternal depression and providing breastfeeding support and newborn health education in a culturally sensitive manner.

Essential Functions and Responsibilities of the Job

  1. Approaches prospective clients at the bedside to introduce the program, verifies willingness to participate, obtains consent, and gathers referral information.
  2. Develops a file for each client recruited into the program, documenting all contacts with the family and agencies and completing data forms and program documentation.
  3. Develops trust and rapport with the clients and provides support using an empathic and client centered approach.
  4. Adheres to all program guidelines, policies and protocols and actively works toward achieving the goals of the program.
  5. Conducts an interview with each client recruited into the program, observing parent infant interaction; assessing involvement of the partner, level of social support, infant feeding and maternal depression; and provides breastfeeding support and newborn health education.
  6. Effectively conducts required client assessments using standardized tools for maternal depression and a risk screening interview. Completes all needed paperwork and data entry for each client in a timely and organized manner, including clear and well written progress notes.
  7. Works closely with Program Director, Program Supervisor, Clinical Coordinator, and administrative support staff to ensure continuum of services.
  8. Collaborates and confers with members of the healthcare team such as the assigned bedside nurse, lactation consultants, social workers, and physicians regarding the need for any additional hospital services or referrals and assists with the referral process if appropriate to ensure a coordination and provision of high quality patient care.
  9. Attends and completes all required trainings, including successful completion of the CLE training if needed. Attends and participates in all home visitation staff meetings, including any group or individual reflective supervision.
  10. Performs other duties as assigned.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
