Salary: The budgeted salary range for the position is $135,958 to $170,333.

Position Summary

The Stroke Coordinator (SC) provides organizational leadership and oversight for the overall care and management of stroke patients within the Comprehensive Stroke Center (CC). In close collaboration with the Comprehensive Stroke Medical Director, the Director of Neurosurgery Operations and other applicable College of Medicine and UIH leaders, the SC ensures that all aspects of the CC program comply with applicable regulatory bodies, national stroke management guidelines (such as: registries, clinical societies), all UIH standards of practice, policies, guidelines and standards of conduct. Responsibilities include coordinating multidisciplinary services such as neurology, neurosurgery, emergency services, diagnostic and interventional neuroradiology, rehabilitation, and critical care. The SC, in collaboration with the stroke team, develops and evaluates care protocols, integrates research into practice, and participates in performance improvement initiatives.

Duties & Responsibilities

Clinical Program Development
  • Works across disciplines and sites of care to ensure best practice is consistent with patients who have experienced a stroke. Including all referral Telestroke partner sites.
  • Assists in providing interdisciplinary awareness of American Stroke Association and Joint Commission standards.
  • Designs and implement processes that contribute to efficient, cost-effective care for patients who have experienced a stroke.
  • Designs and make available pertinent learning tools and resources (e.g., health facts, teaching guidelines and documentation tools, videos, etc.).
  • Assists in outreach efforts to inform the greater community regarding stroke.
  • Coordinates Quality Improvement initiatives of the Stroke with association with COM clinical departments.
  • Collaborates with internal staff or external partners to ensure the accuracy, completeness, and reliability of the stroke-specific data abstraction process and monitoring.

Expert Clinician
  • Attends annual or semiannual national or international conferences to ensure knowledge of stroke-specific best medical practice and new treatments.
  • Provides expert nursing care in an expanded role to patients with stroke.
  • Provides consultation to nursing, pharmacy, medical staff, and others to develop, implement, and evaluate a comprehensive plan of care, including a discharge plan, for patients with stroke to achieve desired patient outcomes.
  • Verbally communicates pertinent information with key clinicians in a timely manner. Utilizes appropriate documentation tools for effective written communication.
  • Intermittently provides reinforcement of stroke education to patients and family.
  • Consults with all team members across the continuum to ensure optimal patient transition plans.
  • Participates in Acute Stroke Code activations to ensure protocols are followed.
  • Provides post hospital contact to evaluate transition and outcome.
  • Provides telemedicine program support with establishing new sites and site support for existing sites.
  • Ensures Highest standards in Patient/Family Education
  • Role models best practice in the area of stroke education, inspiring inpatient clinical staff to actively engage in stroke prevention education.
  • Develops and operationalizes a philosophy of learning for patients and families with stroke.
  • Utilizes an empowerment model to create patient partnerships, employing strategies that foster self-management for stroke prevention.
  • Tailors educational interventions to match the patients' and families' unique learning needs, barriers and preferences (e.g., current level of understanding, developmental status, cultural and language needs, impaired vision or hearing, etc.).

Staff Education/Consultation
  • Consults with clinical staff concerning nursing care in patients who have experienced stroke.
  • Establishes collaborative relationships with physician staff and other members of the team for effective interdisciplinary management within the hospital and beyond.
  • Develops and delivers educational programs for all staff that care for patients who have experienced stroke.
  • Serves as a clinical and operational resource for all UIH Telestroke Partner Sites.
  • Develops and provides stroke-specific educational programs for all UIH Telestroke Partner Sites.

Leadership
  • Be the leader and participate actively in quality improvement projects related to stroke.
  • Collaborate with all Stroke Program Clinicians, Nurse Managers, ancillary services, and educators to ensure that clinicians understand their role in the care of patients with stroke and take accountability for it.
  • Works collaboratively with other nursing peers to improve patient care and advance nursing practice.
  • Collaborates with all applicable nursing leadership for the successful implementation and functioning of the stroke-specific advanced practice nursing standards of practice to ensure successful APN integration.
  • Takes the lead or assists in developing and evaluating written policies and procedures pertinent to the program.
  • Actively participates and represents UIH in internal, local, regional, and national advisory groups to work to advance the science of care for the person experiencing stroke. Examples include, not limited to: Region XI CFD, American Heart Association / American Stroke Association, Illinois Department of Public Health.
  • Participates in professional organization activities.
  • Promotes cost-effective and efficient clinical practices and behaviors in staff throughout the continuum of care.
  • Collaborates with UIH leadership to ensure the successful development of the Telestroke program and, together with the Stroke Medical Director, produces opportunities to collaborate shared institutional quality goals and strategic development.
  • Participates and represents UIH in stroke-specific advisory groups at UIH Telestroke Partner Sites.
  • Develop strategies that support the achievement of program-specific and institutional goals, including optimizing the transfer process for stroke patients, reducing length of stay, and identifying and onboarding new Telestroke partners.

Research, Publication and Presentation
  • Actively participates in research that impacts the patient who has experienced stroke and/or their family.
  • Integrates new knowledge about disease management into practice.
  • Designs and conduct evidence-based practice and quality improvement activities related to the nursing care and management of patients with stroke.
  • In collaboration with researchers and clinical experts, evaluates effectiveness of select initiatives.
  • Advances the practice of nursing through publications and presentations.
  • Other Duties as Assigned


Minimum Qualifications
  • Bachelor of Science in Nursing Required, Master of Science in Nursing Preferred.
  • Minimum Two (2) years of relevant clinical nursing experience.
  • Experience as a nurse provider within a Joint Commission certified stroke program Licenses & Certifications.
  • Registered Nurse, Licensed in State of Illinois.
  • Knowledge of educational principles as they relate to patients and families.
  • Demonstrated excellence in patient and family education and self-care management.
  • Knowledge and experience in development, implementation, and evaluation of clinical programs.
  • Demonstrated leadership abilities.
  • Excellent written and verbal communication skills.

Preferred Qualifications
  • Excellent interpersonal communication, problem solving, and conflict resolution skills.
  • Evidence of ability to function with a high degree of independence while actively collaborating with health care members providing care for highly complex patients.
  • Knowledge of multiple computer applications including word processing, databases, and PC based presentation software.
  • Knowledge of computer skills including Word, Excel, and PowerPoint.
  • Knowledge of process improvement methodology and tools Knowledge of data collection methodology.
  • Knowledge of basic statistical process control components Knowledge of benchmarking tools and best practices Problem-solving and team facilitation skills.
  • Strong analytical skills.
  • Ability to train, present to small and large groups.
  • Ability to act as a consultant to clinicians and non-clinicians.
  • Excellent customer service skills.
  • Ability to work together and well with other team members.
  • Ability to work well under stressful situations.
